We are that kind of non-profit mental health care provider! HeartLand Child & Family Services offers community-based mental health services to children and young adults at our clinics in North Sacramento and Arden/Arcade. We are seeking a Family Partner / Peer Advocate for our FIT (Flexible Integrated Treatment) community-based mental health treatment program.

Minimum Qualifications: (Applications who no not possess these minimum qualifications will not be interviewed)

- Personal LIVED EXPERIENCE as the PRIMARY CAREGIVER for a child with mental health challenges (e.g. child receiving services from a mental health provider, alcohol and drug services, involvement in child welfare system and/or juvenile justice system). Applicants must meet the accepted definition of "mental health consumer" for the state of California.

The ability to consistently work 7.5 hours each day (half hour meal break), 40 hours a week, Monday-Friday, during business hours and adhere to an assigned work schedule in the office and community.

Availability to occasionally work evenings and weekends, as program and client needs dictate.

High School diploma or equivalent.

Basic computer literacy skills.

Reliable personal transportation readily accessible throughout the workday with a valid class "C" California driver license and an auto insurance policy meeting the minimum legal standards for the state of California.

Ability to pass a California DOJ criminal background screening.

Any combination of education, training, and experience necessary to perform the job duties for this position.

SKILLS: Must be a collaborative team member, organized, computer literate, possess good time management skills, and proficiency in Microsoft Office applications. Experience with AVATAR or other EMR (Electronic Medical Record) highly preferred, but can be trained/learned. Ability to communicate exceptionally well, both orally and in writing. Must be able to engage parents, caregivers, multidisciplinary team members, and professionals within the complex systems of care. Family Partners/ Advocates share their personal life experiences and unique perspectives in a culturally competent and strength-based manner, making services more accessible to youth and their families by informing services and improving systems of care.

RESPONSIBILITIES (Not all inclusive):

The Family Partner / Advocate will provide assistance and support to parents and caregivers (in person, phone, and telehealth), mentoring, community and home-based support, accompany youth and parents to meetings (i.e. IEP, 504 Plan, SARB, CFT), provide information and link families to community resources and benefits including housing support services, facilitate skill building groups for caregivers, provide education about advocacy to the clinical team, youth, and caregivers and participate in regularly scheduled Child and Family Team Meetings. A major portion of this position requires the Family Partner/Advocate to help engage parents and caregivers in services, and to support them in overcoming barriers, including advocacy in schools as well as with our own mental health treatment team. The ability to ensure that parents and caregivers have their "Voice and Choice" heard and acknowledged is a key function of his position. All services provided must be documented in progress notes written and submitted according to agency and county standards, in timely manner.

QUALIFICATIONS:

At minimum, must have prior lived experience while having responsibility for a child that has received services from a mental health/child welfare/juvenile justice system and be willing to share that experience. The Family Partner/ Advocate must have the knowledge, skills and ability to relate to parents and caregivers, have experience navigating the systems listed above, and be seen as a positive role model.
