Social Worker Job at Lasell Village
Lasell Village is a unique life plan community with education at its heart, located on the beautiful campus of Lasell University in Newton. We are a caring and vibrant community home to more than 225 residents in independent living apartments, supported living studios, and a 5-star skilled nursing and rehabilitative facility.
The Senior Living Social Worker is responsible for identifying the social, psychological, and emotional needs of residents in independent living, and Lasell Studios, and for making recommendations and arrangements for services to meet those needs. The position includes time spent with families and residents in the Wellness Center, as well as documentation, meeting and related conditions. Quality customer service is a top priority for everyone at the Village.
This is a 32 hour a week salaried position. The salary is $73k.

PRIMARY RESPONSIBILITIES:
- Make routine contacts with residents of Lasell Village and Lasell Studios
- Assess resident and family needs, develop resources and appropriate interventions to meet those needs
- Meet with residents on an individual basis as needed to provide support, resources, or care management services
- Participates in resident admission procedures, including “Wellness Center interview” and cognitive assessment
- Assist with transfer of resident from one level of care to another, as needed. Includes facilitating care transition discussions with residents and participating in family meetings
- Documents assessments, results, meetings, and changes
- Run and Facilitate groups as needed to address resident needs and concerns
- Sponsor and or present programs to the community around end-of-life decision making
- Provides in-service training to staff and residents on relevant topics, as needed
- Works cooperatively with Social Worker at Lasell House and Manager of Lasell Studios to provide a continuum of care
- Performs necessary administrative functions, record keeping, correspondence, and communications as needed
- Develops and maintains community resources and outside professional contacts
- Keeps educational credentials and professional certificates current
- Develops goals and objectives annually
- Maintains confidentiality and protects sensitive Protected Health Information (PHI) at all times
- Promote the Mission, Vision, and Values of Lasell Village in all areas of responsibility
SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:
- Masters in Social Work is preferred
- Minimum requirement is a four-year college or university education with a B.A./B.S. degree in group work, psychology, counseling, social work, or related field.
JOB SKILLS:
- Five years’ experience in a social services role
- Must possess excellent communication, leadership, and organizational skills, and have a high degree of flexibility and adaptability
- Must be able to assist residents and staff in case of a disaster
- Must have excellent interpersonal and communication skills
- Thrive in a team environment and work well with others
