Resident Care Coordinator Job at Royal Columbian Senior Living
Purpose:
Resident Care Coordinator will assist in ensuring resident care needs are met as directed by the Health Services Director and Executive Director by coordinating and providing resident care and assisting in the oversight of health services team members, procedures and systems.
Minimum Eligibility Requirements:
· LPN, NAC, CNA or past experience as a Resident Care Coordinator.
· Experience in an Assisted Living setting preferred, and the ability to work on-call, weekends and holidays as scheduled.
· Ability to establish effective relationships with residents, family members, and staff
· Strong documentation skills.
· Ability to represent the facility in a positive and professional manner.
Essential Job Functions:
· Act as a lead/trainer
· Acts as on-call staff and fills in shifts when requested by the Health Services Director
· Checks daily for physician orders and follows up if order is not returned.
· Notifies physician, nurse and / or designated agent(s) of resident status/changes as appropriate.
· Coordinates care with outside agencies.
· Primary Contact for family and resident regarding care implementation needs including requesting or ordering supplies per the resident/family preference.
· Ensures all doctor’s orders have been transcribed correctly in the MAR and ensures all orders have been signed off by the Medication Aide.
· Order and maintain blood glucose supplies, incontinent supplies, house stock and care supplies as well as durable medical equipment as needed.
· Inform HSD when supply stock is low.
· Records resident information as required.
· Update care plan books frequently and when residents move from AL to FW.
· Ensure NEW ADMIT check list is fulfilled.
· Will serve as the day to day coach and mentor to develop care staff and ensure proper execution of their job duties.
· Observes problems, concerns, issues with employees and/or residents and communicates them appropriately to Executive Director and Director of Nursing Services.
· Ensures that any additional communication systems such as bath lists, laundry lists, daily task list, etc., reflect each resident’s person-centered needs.
· Implement interim service plans as care needs change and assure all medication techs are in-services on implementing interim service plans as needed
· Read the 24-hour communication book and respond as necessary to issues.
· Read any new interim service plans that could have been initiated.
· Monitor the medication room to assure organization and cleanliness.
· Ensure all areas are stocked with sanitizer and disinfecting supplies, as well as all COVID supply carts
· General record-keeping and filling as needed.
· Fill in for the receptionist as directed by Executive Director
· Assists with orientation of new staff members to job responsibilities
· Provide Business Office Manager with a Change of Circumstance form when a resident leaves/returns from a medical facility, ASAP.
· Performs other tasks/duties as assigned.
Working Conditions:
- Able to work full-time, on-call, weekends and holidays as scheduled.
Physical/Sensory Requirements:
- Medium work – Ability to exert 20-50 pounds of force occasionally, and/or 10-20 pounds of force frequently, and/or up to 10 pounds of force constantly to lift, carry, push, pull or otherwise move objects.
- Must be able to see well enough to write and read, and to visually interact with residents, families, and staff.
- Must be able to speak and hear well enough to verbally interact with residents, families and staff.
OSHA Category Classification:
II: Potential for exposure to blood-borne pathogens.
Pay will be determined by experience.
Job Type: Full-time
Pay: $16.06 - $26.34 per hour
