HCA Solutions is currently seeking qualified candidates for the Social Worker positions at the Norristown State Hospital. Position is fulltime, 7:30a – 4p (30 min lunch), to start 5/29/23.

Provide support services in areas such as conducting agency specific reviews to ensure consumers are receiving appropriate care that meets State and Federal guidelines. Review written documentation and meet with consumers and stakeholders to evaluate the quality of the services being provided. They will monitor issues related to medical and financial eligibility of the consumer. Statewide travel required as necessary, including some overnights.

Possible Duties:

• As part of the treatment team, will work effectively with the interdisciplinary team members; actively participate in the treatment planning process; inform team of pertinent information, changes, and observations, CSB and family involvement; actively pursue the input and assistance from CSB and relevant other community agencies regarding treatment, pre-discharge & discharge planning. Interpret family and patient issues for other staff members. Make ongoing assessment of patients.
• Independently maintain clinical records on assigned caseload in accordance with hospital regulations. Provide casework services to include identifying and coordinating services and resources, corresponding/contacting AR, CSB, and significant others regarding treatment.
• Independently complete annual psychosocial assessments.
• Pre-discharge planning with ARs to educate them on the discharge process and provide guidance and recommendations on resources in their area of interest.
• Enters timely assessments and progress notes in assigned patients' Clinical Records to assess psychosocial function and treatment effectiveness.
• includes patients and significant others in treatment decisions. Initiates involvement by teleconference if not able to be present or update on patient’s status via telephone.
• Maintains ongoing contact with Community Services Boards (CSB) regarding treatment and discharge planning issues and reports this back to the Interdisciplinary Team. Attends meeting that will impact patient treatment outcome (i.e. attending scheduled commitment hearings, monthly CSB meetings etc.).
• Obtains the necessary authorization of releases of information for planning. Initiates the designation of the authorized representative’s process as needed.
• Works with Reimbursement and unit staff on disbursement of excess resources in patients’ Patient Fund Accounts.
• Successfully works with community agencies and individuals on obtaining guardians for patients.

The Social Worker must possess:

• Ability to individually, and in conjunction with interdisciplinary teams, develop goals and implement treatment plans according to client's needs and problems.
• Ability and skills in providing individual and group counseling as well as family supportive counseling.
• Working knowledge of available social, economic, and community resources.
• Ability to interview and gather medical and psychosocial data and interpret it in oral and written reports.
• Knowledge, skills and ability to communicate effectively, orally and in writing.
• Knowledgeable with NASW Code of Ethics.
• Ability to perform at a fast pace with frequent changes.
• Ability to interact with other professionals as a contributing team member.
• Ability to independently carry out work actions/job responsibilities thoroughly and timely

• Associate degree in Human Services or equivalent education and experience.
• Basic computer skills to include, but not limited to, Microsoft Office Products.
• At least one year of experience.
