$19.00 - $24.09 an hour

In an aim to help keep our patients, staff, and communities safe, STRIDE requires that all hired employees be fully COVID-19 vaccinated and to obtain an annual flu shot. STRIDE CHC employees are also required to be screened for tuberculosis and to complete a PPD test.

What will you be doing in this role? A successful candidate will:

General Purpose: Provide support for maintenance of clinical health records, follows STRIDE protocol for retention, maintenance and release of health records. Process and follow up on internal and external specialty referrals including processing health records.

Essential Duties/Responsibilities:

  • Ensures health related reports/records are scanned and indexed into the Electronic Health Records (EHR) within the required guidelines
  • Ensures patient information is maintained and treated according to HIPAA privacy laws and confidentiality guidelines.
  • Follows STRIDE and HIPAA guidelines when releasing health records to authorized requested parties.
  • Assists and responds to health records related questions and requests
  • Responds to records requests via Patient Portal.
  • Faxes records requests on behalf of STRIDE following policies and procedures
  • Communicates with team members and Customer Care Center management team regarding workload and challenges
  • Assists and responds to referral related questions
  • Routes messages to clinic staff following policies and procedures
  • Informs and guides patients to resources within or outside of STRIDE Community Health Center
  • Maintains communications and collaborates with patient, physician, payer representative and internal staff
  • Forwards referral information to appropriate STRIDE employee following policy and procedures
  • Process internal and external referrals daily, prints and faxes pertinent patient health information to specialists according to STRIDE and HIPAA guidelines
  • Enters referral information into database
  • Follows up on all referrals following guidelines
  • Speak to specialist offices, as necessary, to assist in the guidance of the referral process and problem solve any concerns that may arise as a result of the referral process
  • Pulls and/or requests reports for the specialist to close out the referral
  • Scans and imports specialty reports in a timely manner following policy and procedures

Non-Essential Duties/Responsibilities

  • Attends all staff meetings and all other meetings as assigned.
  • Completes all other duties as assigned.

Qualifications

A successful candidate will possess the following education or formal training:

Education or Formal Training: High school diploma or GED required.

A successful candidate will possess experience:

Experience: 1 years related experience preferred, prior electronic health records experience preferred.

  • Ability to work independently and organize work in a manner that ensures accuracy and efficiency.
  • Ability to analyze and synthesize data to determine the appropriate program.

  • Ability to manage multiple projects to successful completion at a given time.
  • Ability to demonstrate the capability to effectively and sensitively respond to the needs and concerns of the full range of MCPNs diverse patient population.
  • Ability to handle sensitive information ethically and responsibly.
  • Ability to protect the confidentiality of patient, employee and business information

Material and equipment directly used: Computer, printer, multi-line telephone systems, fax machine, copy machine, calculator, and all Microsoft Office products including Outlook, Excel and the internet. Electronic Health Records software.

Working environment/physical activities: Office environment within a call center setting. Enters data into computer programs via computer, mouse and keyboard. Moves about the office environment and occasionally to other locations. Moves/transports objects up to 25 lbs occasionally. Communicates information to others. Discerns/analyzes information from others to assist in decision making. Works independently.
