We are hiring a Physician -Diabetes Care Specialist/Diabetologist at our Lisbon Avenue Health Center in Milwaukee, WI. This is a full-time position and does
not require working late nights, weekends, or holidays.

JOB SUMMARY
Provide comprehensive out-patient clinical and cardiometabolic care to diabetic patients as part of a multispecialty and collaborative team. Delivers care appropriate for an out-patient, primary care community health center with a multicultural patient population.
DUTIES AND RESPONSIBILITIES
- Provide diabetic/cardiometabolic care in an ambulatory care facility in a consultive fashion. Responsible for the delivery of care, treatment, and referrals as needed.
- Evaluate patients, advance needed care with goal of optimizing metabolic control using full complement of available interventions to decrease morbidity and mortality.
- Work in collaboration with onsite primary care providers, ambulatory pharmacy services, care management, dietary consultation, and podiatric services.
- Maintain high standard of medical practice and participate in the training and development of the medical team. Assist in medical education of both the patients and clinic staff.
- Actively participate in clinical quality performance and improvement.
- Facilitates the patient's transition within and between health care settings; refer and transfer patients back to primary care, provide follow-up care and discharge patients.
- Participate in the development of clinic protocols by making recommendations on relevant cardiometabolic topics.
- Counsel patient regarding medical and other problems which might affect patient's health and well-being.
- Provide direction and leadership to the clinic's medical support staff, with regard to the administering of a patient's care.
- Perform other job-related duties, as may be assigned, by the Chief Medical Officer.
EDUCATION, TRAINING AND EXPERIENCE
Must be a graduate from an accredited School of Medicine and Residency Program. Currently licensed to practice in the State of Wisconsin. Have a thorough knowledge of the principles and practices of medicine commensurate with his/her training. Board certification required with subsequent fellowship in Endocrinology or certification in cardiometabolic care.
It is mandatory that all employees, employed providers and volunteers are vaccinated. All hired candidates must submit proof of vaccination prior to their orientation. This is a mandatory requirement for Progressive Community Health Centers.
