Position Summary: This position reaches out to and schedules appointments for patients needing preventive and chronic illness follow-up services, patients newly assigned to Infinity Health to establish care, and patients requiring follow-up regarding Medicaid eligibility. This position coordinates patient eligibility and assignment with Coordinated Care Organizations (CCO’s), their Risk Assuming Entities (RAE’s), and the State, and provides coordination of other membership services and needed for Infinity Health patients and as specified by their supervisor.

Essential Duties and Responsibilities:

  • Review clinical outreach, membership rosters, and other reports as well as the patients’ electronic health record to determine the outreach required.
  • Perform outreach to patients and schedule appointment.
  • Track outreach contact to patient in the electronic health record, and notify the patient’s team when follow-up by the patient’s team is required.
  • Work with other Membership Services staff to ensure that all patients are accurately documented as Infinity Health patients in the electronic health record.
  • Participate in process and systems improvement activities and trainings to ensure that Infinity Health provides outstanding membership services for our patients.
  • File required paperwork with the CCOs, RAEs, and the State to insure accurate attribution of our patients with Medicaid.
  • Help orient team members to outreach workflows and provide support and back up to other team members as needed.
  • Assist in updating workflows.
  • Provide membership outreach services for patients of assigned care teams and attend care team meetings as appropriate.
  • Travel to assigned care team sites as needed.
  • Perform other duties as assigned.
  • Handle protected health information in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Qualifications:

· BA or BS in a human service or similar field or CNA, CMA, LPN or RN nursing license

· Health care and/or human service experience preferred.

· Knowledgeable of community resources and willingness to develop relationships with external agencies.

· All employees are required to be fully vaccinated for COVID-19 prior to employment or must request an exemption no less than 10 days prior to start date.

Job Type: Full-time

Pay: From $15.00 per hour
