GENERAL DESCRIPTION OF ROLE AND DUTIES

The Care Management Housing Coordinator is a vital member of the Care Management team. The Housing Coordinator's primary responsibility is to provide services to low income individuals living with HIV/AIDS in order to increase housing stability, decrease the risk of homelessness, and increase access to care and medical support services. This will be accomplished by working directly with clients, assisting them in identifying their needs, referring them to appropriate resources to assist with these needs, determining eligibility and financial support according to Ryan White (RW) and Housing Opportunities for Persons with AIDS (HOPWA) grant regulations and policies and facilitating their utilization of resources.

The HCHC Care Management Medical Case Manager is responsible for carrying out the mission of Healthy Community Health Centers by providing quality support services to persons infected and affected with HIV/AIDS. This is accomplished through collaborative and client-centered needs assessment completion, goal setting, supporting, and coordinating appropriate referrals to specialists and community-based resource programs to ensure access to appropriate health services, medications and other available community resources.

Requirements:

Minimum education requirements are:

  • Minimum educational requirements: high school diploma with at least 1-year related experience; bachelor’s degree in human services or a 2-year degree in a Human Services related field preferred

Experience requirements:

  • Excellent oral and written communication skills
  • Excellent interpersonal skills
  • Strong computer skills, including word processing, spreadsheets, database programs, internet.
  • Ability to work effectively with a diverse population and knowledgeable about and demonstrates the ability to be culturally competent
  • Ability to plan, organize, prioritize multiple tasks, and accomplish goals in a timely manner
  • Ability to work under stressful and difficult emotional situations
  • Experience working with and accessing community resources
  • Ability to work independently and as a team member
  • Have own transportation with full liability insurance

Additional Skills Desired:

  • Bilingual/Multilingual
  • Knowledge of medical terminology, procedures, insurance and government health programs (SSI/SSD, Medicare/Medicaid, and Department of Social Services)
  • Health Information Portability and Accountability Act (HIPAA) knowledge
