Family Support Counselor (Home To Stay Care Manager Job at Peace River Center

This is a bachelor level position and is designed to assist with successfully reunifying children with their parents in Circuit 10 after removal due to abuse, neglect or abandonment by providing intensive in home services. The Care Manager works closely with the parents and the child/children in their homes and in the community to assist where needed in order to have a successful reunification with their child and prevent further removal. Families are typically open with the Home To Stay program 7 to 10 months.

Essential Duties:

· Ability to provide counseling, in-home parenting, anger management, crisis intervention/safety plans, substance abuse education, domestic violence education, stress management, financial planning, employment assistance, assistance with hygiene issues/hazardous conditions as needed to the families.

· Ability to assist family in attaining goals for successful reunification.

· Ability to conduct family assessments/interventions including participation in Family Team Conferences.

· Ability to maintain linkages and establish good rapport with the Department of Children and Families/Heartland for Children, and other community agencies within Circuit 10.

· Ability to utilize support funds as needed.

· Ability to attend all scheduled work hours, meetings, trainings, and other center functions, including on-call as required.

· Ability to perform quality work within deadlines with or without direct supervision.

· Ability to interact professionally with other employees, customers and vendors.

· Ability to work effectively as a team contributor on all assignments

· Ability to work independently while understanding the necessity for communicating and coordinating work efforts with other employees and organizations.

· Ability to meet state mandated background check and screening requirements.

JOB DESCRIPTION

Care Manager- Home to Stay

· Employees offices will be located at the same PRC location where they should work to begin and end their day, unless their day begins or ends in the field.

· This is a 24/7 program so designated schedule may be flexible and vary.

Qualifications:

Education:

Bachelor's degree in social work or human services related field

Experience:

Minimum of two years' experience in family-centered practice required

Certifications:

None

Training:

Upon hire, 40 hours of contract training is required and must be completed prior to providing direct services to families

An additional 40 hours of enhancement training annually including all PRC required training

Knowledge and Skills:

Excellent written and verbal skills

Good interpersonal communication skills

Good organizational skills

Ability to work with diverse populations and diverse family structures

Ability to provide counseling to children, youth, adults as well as family counseling

Flexibility to work independently

Ability to work within a flexible schedule including weekends

Good Time Management Skills

Safety Equipment:

Universal precautions

Comply with Occupational Safety and Health Administration (OSHA) rules and regulations

Life Safety Equipment (fire extinguisher)

Reasoning Ability:

Ability to solve practical problems and deal with a variety of variables

Ability to make quality decisions with limited amount of supervision

Transportation:

Clean driving record

Must be able to meet DOT driving requirements of age 21 and possess a valid Florida's driver's license.

Provide and maintain proof of insurance for bodily injury coverage of $50,000/$100,000.

Must have reliable transportation.

Machines, Equipment, Tools:

Computer, Telephone, Fax, Calculator, Cell phones, Air Cards/Lap top, Copy Machine

Lap Top/Air Cards and cell phones will be used in the field . Equipment will not be for personal use. Employees will utilize the Main Street printer and fax to print and send required documents.

Work Environment:

Fast paced work environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. The noise level in the work environment is usually moderate. Must understand and accept the possibility of exposure to inside environmental conditions such as communicable diseases and physical/verbal abuse from an out of control patient/client.
