GENERAL SUMMARY:

The Teen Center Clinical Social Worker collaborates with members of an interdisciplinary team to assist patients and families with complex psychosocial needs. Corewell School Based Health Centers and Expanding, Enhancing Emotional Health Programs are located on school grounds. Corewell’s Teen Health Center-Taylor is a school linked program which is free-standing. Staff members work closely with school administration to provide interventions to students during the school day. These interventions may include, but are not exclusive to clinical therapy, crisis intervention, group therapy, and psychoeducation groups.

ESSENTIAL DUTIES:

  • Conducts psychosocial assessments and treatment planning with patients and or families to assist with identified psychosocial needs.
  • Provide short- and long-term therapy to students enrolled within the school.
  • Completes and documents the assessment and plan in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, and psychosocial imperatives.
  • Provides community resources to patients and families with appropriate follow-up.
  • When appropriate, works with school administration and clinical staff to develop a plan of care.
  • Provides clinical and professional consultation regarding teen center patients to other teen center staff.
  • Demonstrates knowledge of policy and procedures and regulatory compliance to optimize client care and safety.
  • Supports performance improvement initiatives based on patient satisfaction and quality outcomes.
  • Participates in recruiting, orienting, and mentoring new staff.
  • Maintains professional affiliations and memberships.
  • Demonstrates responsibility for own professional growth and development.
  • Uses EPIC for patient documentation.
  • Provides mental health evidenced based curriculum to students
  • Responsible for tracking data for grant reports; assists team members as needed.
  • Conducts mental health evidenced based prevention programs.
  • Provides mental health education and training to schoolteachers and administration, teen center staff, and the community.

STANDARD QUALIFICATIONS

  • Education / Training:
  • Master’s degree in social work
  • Work Experience:
  • Experience with youth in clinical therapy and crisis intervention.
  • Certification, Licensure, Registration:
  • State of Michigan Social Work License (LMSW)
  • Other Qualifications:
  • Demonstrate knowledge of growth and development and dynamics of behavior for age related groups. Must demonstrate excellent oral and written communication skills. Must be proficient in Microsoft Word and ability to adapt to technical demands of electronic communication. Ability to work independently but contribute positively to team performance. Ability to manage, prioritize multiple situations and tasks while remaining calm. Must demonstrate willingness to be flexible and cope with multiple job stressors.
