Home Care Scheduler – Full Time Personal Touch Home Care

Personal Touch Home Care Services is a leading national home care agency.

We are seeking an experienced individual for our position as a Scheduler who will be responsible for coordinating, scheduling patient visits, case assignment, coverage and scheduling under the supervision of the Director of Clinical Services

Responsibilities for this position include:

  • Schedules for all disciplines: SN, PT, OT, SLP, HHA, MSW, coverage (weekend, vacation, sick and days off) into Electronic Medical Record (EMR).
  • Confers with nurses regarding changes in visits and data enters updates into EMR
  • Assigns therapists via contracted vendors.
  • Discharges patients schedule in EMR
  • Notification of appropriate staff and departments when cases are not opened as scheduled.
  • Keeps Director of Clinical Services and/or Clinical Manager informed of status of case assignments.

Qualifications:

  • High school diploma or GED equivalent or other credential required for the position.
  • Two (2) years experience as home care scheduler or other related heath field.
  • Excellent verbal communication skills and telephone manner.
  • 1 year + CHHA or LHCSA experience preferred
  • 2 + years of experience in healthcare services.

Pay: $40,000.00 - $45,000.00 per year
