We are seeking an RN to join our independently owned and operated national agency. Someone who can help maintain and grow a new business as well as work with the team to increase our client census. A vital component of this position is to communicate the agency mission and philosophy and ensure that our clients better understand the full potential of our services. The successful candidate will also be responsible for collaborating with the owner to improve client support, enhance client satisfaction and maintain healthy care provider morale.

As a Registered Nurse with SYNERGY HomeCare, you will:

• Coordinate and Collaborate care with various Physicians, Peers, Client families, Office Staff, and Medical Entities
• Ensure compliance with state guidelines and company policies and procedures for office and field operations
• Assist with CareGiver trainings, utilizing your expertise as a registered nurse
• Provide valuable advice on how to make cares more inclusive for all
• Provide reliable trainings to the community as a professional RN

• Initiate and complete non-medical homecare assessments and reassessments for comprehensive clients and modify those plans as needed

• Visit clients homes in the following areas: WSP, Eagan, Inver Grove Heights, Bloomington Rosemount, Farmington
• Availability to answer on-call questions on a PRN basis would be a plus

Required Qualifications:

1. Current MN State RN License

2. Home Health/Home Care experience a plus
