East Haven Nursing & Rehabilitation Center is a 200-bed facility servicing the North East Bronx Community. East Haven offers short term rehab, sub-acute care and nursing home care, as well as offering in-house hospice care. Caring is our Commitment is our motto and we are seeking MDS Coordinator with the same dedication.
MDS Coordinator must have solid knowledge and expertise in the MDS OBRA and PDPM process including governmental and state requirements and regulations. The MDS Coordinator assumes a role to achieve optimal and accurate reimbursement by conducting and coordinating the development and completion of the resident assessments. Must be able to maintain and periodically update written policies and procedures and be comfortable educating interdisciplinary team members.
Minimum Qualifications:
- Must have long term care experience
- Must have experience in MDS
- Must have a valid New York RN license
- Previous experience as a RN supervisor preferred
- Computer literate
- Sigmacare Knowledge preferred
