What you will do in this role:

• Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical, financial, and psychosocial approach through the continuum of care.
• Performs concurrent clinical review to determine appropriateness of admission and continued need for care delivery in the acute care setting.
• Performs telephonic clinical reviews with claims insurance personnel to facilitate reimbursement for the delivery of patient care services.
• Assesses the patient for discharge planning needs and referrals to the interdisciplinary team.
• Supports collaboration and communication among the patient/family and entire health care team regarding the patient’s plan of care, continued needs post-discharge, and facilitates those services required.
• Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization.
• Identifies quality and/or risk issues and reports these to the appropriate individual/department.

What qualifications you will need:

• Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure
• BLS certification through AHA or ARC
• Graduate of an accredited school of nursing; BSN preferred
• Three – Five (3-5) years recent clinical or case management experience in area of specialization
