JOB TITLE: Residential Case Manager
Reports to: Residential Director/Case Management Supervisor
Position: Full-time
Here’s an overview of what I do:
This is a full-time position responsible for assisting in the assessment, development, and progress of the residents as they move through our restoration model. This role will be active in the individual restorative plans for each resident. It also includes mentoring and coaching the residents as well as the residential coaches who work with the residents.

Requirements for this position:
- Agreement with and living out our statement of faith.
- Driver’s license and use of your own car, if/as needed.
- Must pass state-wide criminal background check.
- Maintain confidentiality and other responsibilities listed in governmental laws and regulations as well as HR4W policies and procedures.
- Due to the cliental we serve, this position is only open to females.
- Bachelor’s Degree in social work, psychology, human services, or equivalent field preferred, and/or at least two years’ experience serving at-risk populations.
Here are the “big” things I’m working on all the time:
- Development and implementation of Individual Restorative Plans for residents in the home o Help residents assess their needs and set goals in each of our program’s eight dimensions, and write their individual case plans o Meet individually with residents once a week to assess progress, adapt goals, and mentor and coach residents as they walk through the challenges of healing and freedom individually and within the home with staff and other residents o Maintain accurate records, and keep residential staff apprised of residents’ progress
- Assist residents in identifying and acquiring medical, financial, therapeutic, educational, vocational, and social resources in the community o Maintain communication with community providers and monitor residents’ compliance with individual case plans o Assists residents in following through with individual case plans with community providers, including follow-up appointments, acquiring prescribed medications, accessing records and other resources, and other plans of action
- Work alongside and provide support to Residential Director in monitoring the overall health of the individual residents as well as their interaction within the larger community of HR4W. o Provide direction and support to residential coaches o Address conflict and facilitate conflict resolution o Assess and manage risk appropriately o Assess and address resident’s compliance with HR4W’s Mutual Covenant
Agreement o Assist with intake interviews and assessment processes, as well as exit procedures. Access records from prior medical, therapeutic, residential homes and substance abuse facilities to aid in intake assessment.
