We are looking for a professional, compassionate, and caring Personal Care Coordinator to join our Fellowship Connected Living team. In this role, you will assist independent living seniors using the Livindi platform throughout New Jersey.

Position Highlights

• Administers bedside and personal care such as showers and grooming
• Prepares and maintains records of client progress and services performed, reporting changes in client condition to manager or supervisor
• Performs housekeeping duties such as meal prep, tidying up living area, and laundry
• May perform health care-related tasks such as monitoring vital signs under the direction of the registered nurses
• Transports clients to locations outside the home such as to physicians' offices or on outings, using client's motor vehicle

Requirements

• Must enjoy working with seniors with ability to provide exceptional care
• Minimum 1 year healthcare/home care experience
• Valid NJ CHHA certification
• Strong communication and customer service skills to communicate with and respond effectively to instructions and requests from clients/clinical team

• Valid driver’s license and reliable transportation
• Must drive to multiple clients throughout the day

Job Type: Full-time, part-time and per-diem

Compensation: Up to $23 an hour
