Patient Care Coordinator Job at SOUTH PLAINS RURAL HEALTH SERVICES, INC
The Patient Care Coordinator is responsible for facilitating case management services for all patients with a chronic condition.
SPECIFIC DUTIES AND RESPONSIBILITIES:
1. Makes referrals, appointments for medical services for children and adult patients of the community health center.
2. Collection and review data for analysis of chronically-ill patients in accordance with protocol and distribution of data to providers for action.
3. Development and implementation of outreach efforts to facilitate patient compliance; tracking patient’s ER visits and follow-up care.
4. Identifies all appropriate available community resources; and supporting care management and follow-up.
5. Develops and implements all medical referral services for all medical patients of the center as directed by the Provider
Must be Bilingual.
