On Call Nurse/ Triage Job at Utah Home Health and Hospice

$60 - $250 a day
7 days on, 7 days off. Utah Home Health and Hospice is looking for dynamic, motivated, and compassionate RN to join our patient-centered care team for on call nursing.The primary responsibility of the home health and hospice RN is to provide direct support to the patient and their families while functioning within a medically approved plan of care.
Principle Duties and Responsibilities:
- Work as part of an interdisciplinary team that includes physicians, nurses, social workers, chaplains, massage therapists, certified nurses’ aides, etc.
- Work with primary care physicians and the hospice medical director.
- Accept and understand the mission of our home health and hospice.
- Ensure that the educational needs of the patient and family are met.
- Review and document patient needs in care plans, progress notes, etc.
- Perform on-call responsibilities as needed by the organization
- Complete initial assessments
- Travel required
Qualifications:
- Must possess current, unencumbered, active license to practice as a Registered Nurse (RN)
- Must have valid car insurance and driver’s license.
- Knowledge of and experience in pain and symptom management.
- Well-developed physical assessment skills.
- Must be able to read, write, speak, and understand the English language.
- Must possess patience, tact, and a positive attitude.
- Hospice/Home Health experience preferred.
Job Types: Full-time, Part-time
Pay: $60.00 - $250.00 per day
