Job Information
Performs assigned duties, under the direction and direct supervision of the Pharmacist and in compliance with and as allowed by the California State Board of Pharmacy.
ESSENTIAL DUTIES
- Assists in gathering information and completing patient medication reconciliations in emergency department & acute nursing. This involves communicating with patients, pharmacies & providers.
- Coordinates with case manager, providers & pharmacist to provide ‘meds-to-beds’ before patients leave the facility or delivering them to patient’s homes.
- Perform patient medication centered calls.
- Assists pharmacist with the preparation, distribution, out-date monitoring, and accurate inventory control of medications throughout the facility.
- Supports and participates in hospital wide quality initiatives
- Adheres to regulatory and compliance standards
Qualifications
Minimum Education (or substitute experience) Required:
- High School or equivalent.
- Pharmacy Technician License with the State Board of Pharmacy
- BLS certification
Minimum Experience Required
- Three (3) years of experience in an outpatient or hospital pharmacy
Experience Preferred
- Knowledge of EMR system or ability to learn and use the district EMR.
- Hospital pharmacy experience
