The Registered Dietitian is responsible to the Director of Clinical Services to provide consultation and instruction in appropriate nutrition for clients. Assess client's' nutritional status and recommend special nutritional interventions to the physician's Plan of Care.

Educational Requirements

Bachelors degree or other advanced degree in dietetics. Registration with the Commission on Dietetic Registration, the credentialing agency of the Academy of Nutrition and Dietetics; and license or certification by the state where practices, if required.

Qualifications


  • American Dietetic Association registration.

  • Current knowledge of principles, concepts and methodology of nutrition appropriate for ill patients.

  • Excellent communication skills both verbal and written.

  • Ability to work well with an interdisciplinary team.

  • Ability to develop and maintain rapport with patients and families.

  • Demonstrate problem solving and decision making abilities.


Experience


  • At least one year previous experience, preferably in home health care or hospice.


Travel


  • 100%; necessary on a daily basis.


Working Conditions


  • Exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive patients.

Job Specific Details:


General Responsibilities



  • Assesses the nutritional needs of patients upon request.

  • Determines the effectiveness of medically prescribed diets.

  • Teaches patients/families and staff special dietary regimens and nutritional requirements as appropriate to diagnosis and individual preference.

  • Documents patient care in the clinical record.

  • Identifies factors relating to variances of weight and/or size.

  • Attends IDT/IDG care meetings as appropriate.

  • Provides in-service education to hospice staff and educational tools for patient/family.

  • Provides counseling to adapt diet to the patient?s changing status.

  • Consults with the IDT/IDG members and/or RN.

  • Assesses patient/family response to care including access to food/nutritional supplements, ability to prepare food, adequate tools and space and nutritional risk.

  • Collaborates with the nursing staff in applying a nutritional risk assessment to determine the need for a mechanically altered diet and to assess the effect of end stage disease on hydration and nutrition.
