Position: Clinical Coordinator Palliative Program

Location: - Monterey, CA

Job Type: - Full Time

Reports to: Palliative Program Manager or Director of Patient Services

Job Summary

Is responsible for identifying and coordinating patient/family nursing care needs and providing supportive care to patients, families and caregivers. Conducts staff training as needed. Directs and coordinates care of palliative patients including the appropriate placement of the patient under VNA & Hospice service lines based on patients changing needs. Utilizes the nursing process to provide comfort, care and maximize the quality of life for the patient and family.

Education/Experience/Skills/Qualifications

· BSN preferred. Two years of experience as a professional nurse in a medical-surgical facility within the last three years preferably with one year in palliative nursing or two years of recent home care experience with one year in palliative home care.

· Possession of a license to practice as a Registered Nurse in the State of California.

· Possession of current CPR certification.

Special Requirements

· Commitment to the interdisciplinary approach to care. Strong communication skills. Current California driver’s license and car insurance. Ability to read maps.

· Current knowledge of supplies and procedures used in the care of cardiac patients. Operation of required equipment.

Physical Requirements

· Ability to: Sit/stationary standing (for prolonged periods of time), crouch (bend at the knee), stoop (bend at the waist), climb (e.g. stairs), reach (extension and overhead), push/pull up to 50 lbs., ability to be mobile (in office, from office or home, from site to site), kneel, twist/turn/pivot, grasp, lift carry up to 50 lbs., speak (in an understandable voice with sufficient volume to be heard in a

· conversational distance, on the telephone, or addressing groups).

· Acuity to: collect and interpret data, reason, establish facts, make valid judgments and decisions, evaluate, define problems, draw valid

Sensory Requirements

· Ability to: talk and hear (in person/on the telephone), vision (for close work/to read printed materials/long distance), touch (to perform various procedures).

Associated Environmental Factors

· Possible exposure to: fumes, dust, blood and other body fluids, chemicals/cleansers, smoke (cigarette), foul smells, and household pets.

Job Type: Full-time

Salary: $55.00 - $60.00 per hour
