Maplewood Senior Living’s new Inspīr brand of world-class, urban senior living properties is now hiring a Registered Nurse for a Care Manager (Case Manager) role at its new Inspīr | Carnegie Hill property. Located on the Upper East Side, this 23-story, 215-unit residence offers assisted living, enhanced care and memory care. This is an exciting opportunity to be a part of a brand-new development, the first of its kind in New York City. This role will be a critical part of the team in preparation for opening.

Essential Functions:
- Performs initial, scheduled, and Change of Condition assessments on residents and prospects at the direction of the Director of Integrated Care.
- Collaborates with community-based teams and other involved providers to develop safe, highly individualized and outcome-driven care plans for each resident based on assessment findings
- For initial assessments on prospects, the Assessment Nurse works closely with the Director of Integrated Care, the community leadership teams and the corporate clinical and operations teams to determine appropriateness each prospect for potential move-in and to develop effective transition plans to ensure smooth transition to the community
- For assessments on residents in other settings (ED, inpatient, SNF), works closely with Director of Integrated Care, community leadership teams, and corporate clinical and operations teams to determine appropriateness for return to community and develop effective transition plans to ensure smooth transition back to the community
- Participate in case conferences, family meetings, resident rounds and/or other meetings relevant to resident care
- Enters all assessment data into EHR and performs a clinical handoff to resident care team
- Act as a clinical resource and mentor to MSL staff and other members of the resident care teams demonstrating sound clinical knowledge, skill, and expertise and providing consultation and support.
- Helps to foster clinical excellence, accountability, and reliability among team members.
- Ensure complete, timely and appropriate documentation of visits and activities within MSL’s EHR and external EHRs as indicated
- Maintains productivity and quality standards
- Assists with orientation, training, and onboarding of nursing and resident care staff
- Follows all policies, procedures, and practices to meet MSL standards and regulations as set forth by the state
- Assists the nursing leadership with guiding each Community’s wellness staff. Encourage an active and healthy aging program that impacts the quality of resident’s lives, allowing them to have their medical, emotional, and social needs met
- Supports hospitality, leasing, and sales processes with promotion of positive image of Maplewood properties
- Assists other team members for the growth and development of Maplewood Senior Living.
- Develops and maintains professional relationships with coworkers, residents, resident family members, and vendors.
- Is readily available and accessible via phone, email and text during working hours and respond to requests promptly.
- Actively participates in the evaluation of own performance and initiates relevant continuing education to maintain competency
Education/Experience/Licensure/Certification
- Registered Nurse, preferable with advanced training and certification(s)
- Previous experience as a Resident Services Director (RSD)
- Proven and progressive experience within the assisted living communities
- Minimum three (3) years clinical experience working as a nurse with seniors aged 65 and older
- Thorough understanding of geriatric care as well as Alzheimer's disease and related dementias
- Excellent communication, organization and time management skills
- Excellent computer, EHR and software skills including proficiency with MS Word
Maplewood Senior Living conducts pre-employment screening including background check, drug screening, and reference checks.
