ACT Case Manager (Client Opening - AuSable Valley Community Mental Health Authority) Job at Rehmann
The goal of this position is to work as a member of the ACT team to assist persons with severe and persistent mental illness. The ACT case manager will assist to effectively manage the symptoms of the consumer's illness, maximizing their level of integration into the community. ACT case managers/RNs assist in fostering the highest possible quality of life within the consumer's community, decreasing the need for treatment in more restrictive settings. These functions are to be carried out in accordance with the components and principals of ACT fidelity model as well as the policies and procedure of the board.
Requirements

- Bachelor’s Degree (B.A.) in human services field
- One year experience in the mental health field
- Certified as a QMHP, QIDP, and/or CMHP and/or be limited or fully licensed in the State of Michigan in their relative field
