Summary:
The Van Driver is responsible for the safe transportation of customers, day services client, and/or employees to and from the Center during approved activities to pre determined destinations.

PRIMARY RESPONSIBILTIES AND ESSENTIAL FUNCTIONS:
  • Stay with patient or resident, when indicated, at appointment, and as needed;
  • Ensure patient/resident is ready to leave at designated times and is clean and appropriately dressed;
  • Maintain driver’s license in good standing and provide copy of driver’s license and any other necessary documents to Administrator upon request;
  • Participate in training and return demonstration on van, bus, wheelchair lift and/or any other equipment related to transporting prior to starting to drive and upon request thereafter;
  • Maintain CPR and other certification – notify Director of Staff Education at least three months prior to expiration;
  • Maintain mileage and use logs for center owned vehicles;
  • Maintain center owned vehicle in a clean, safe manner; notify Administrator and Maintenance Director of any indication that vehicle may need service;
  • Check and/or inspect equipment that is stored on vehicle for emergency use and notify Administrator of any noted concerns;
  • Ensure appropriate personnel are staffed and available to assist in transports as necessary;
  • Communicate concerns to the appropriate center individuals (i.e. Unit Manager, ADON, DON, Administrator);
  • Transfer patient/resident documents to consultant offices when arriving for appointments and to center nurses following appointments;
  • Use lift and vehicle in safe manner at all times, documenting and notifying DON or Administrator of any concerns or incidents that occur that are out of the ordinary or create a situation which may affect the patient/resident or public;
  • Interacts in a positive manner with customers, and families as a representative of the center;
  • Performs other duties as requested.
EDUCATION and REQUIREMENTS:
  • High school diploma or equivalent
  • Ability to read, write, and follow instructions
  • Certification as a licensed CNA Required
