Title: Licensed Therapist/Social Worker

Supervisor: Clinical Director

Summary Job Description:

Licensed clinical social worker with experience in mental health, family supports and care coordination. Linkage development with community ‘care partners’ and coordinate Member engagement in these partnerships. Aftercare planning utilizing the structured APPLE planning process with all Members is a primary responsibility.

Specific Responsibilities:

  • Coordinate new admissions to program with Admissions and Program staff
  • Provide individual and group sessions
  • Participate in the treatment/recovery planning process with program and staff members
  • Provide documented case management services to members and families including:
  • Coordination of financial eligibility and benefits
  • Coordination of family contact and support
  • Conduct utilization and member care reviews with MCO’s and other payers
  • Oversee Level of Care changes and documented aftercare planning with members
  • Communicate with both referral services and hospitals to facilitate member return to program in a well-coordinated manner
  • Complete authorization reviews with MCO’s and Payers
  • Provide trauma-informed, recovery oriented and Dialectical Behavior Therapy model of care. We will train in DBT when needed.
  • Develop with members the APPLE transitional planning tool in collaboration with the clinician and treatment team
  • Follow up contact with graduated members
  • Consult with families, psychiatrists, therapists for assigned members
  • Prepare and provide group services at assigned program – Transition Workshop
  • Participate in phone coaching/crisis management system which includes:
  • Orient member and family to phone coaching system
  • Carry and respond to coaching phone calls on assigned shifts
  • Attend Team meetings, consultation team and staff meetings
  • Back Up Support to Clinical Director (coverage)
  • Complete continuing educational requirements as aligned with regulatory compliance
  • Other duties as assigned by Clinical Director

Qualifications:

  • At least a Master’s degree in social work or related fields, three years’ clinical experience and case management with psychiatric population, financial eligibility, family support and collaboration of care
  • Demonstrated ability to work in a collaborative team model
  • Strong assessment and documentation skills
  • LCSW/LPC

Position expectations:

  • Staff member will participate in a multidisciplinary treatment team and will support and implement interventions and directives as directed by the Program Director.
  • Staff member will demonstrate in their work with Members at ALL times, compassion, and concern when supporting a Member through embracing our organization's Mission and Core Values.
  • Staff will approach Member engagement from a non-judgmental stance understanding that a Member's behavior is driven by past experience, which may include trauma.
  • Staff will treat and speak to Members with supportive kindness even when a Member demonstrates intense behavioral or emotional actions. Staff will show Members dignity and respect for their values and lifestyle.
  • Staff will seek out appropriate support, consultation with and supervision by the Program Director when they are uncertain about how to respond or support a Member effectively.
  • Staff will report back to the treatment Program Director and the Team, any observations of Member behavior that suggests Member may need additional treatment interventions and/or support.
  • Staff will engage with all external parties/individuals with appropriate professionalism and with a positive customer service approach, understanding that they are a professional who is representing the organization at all times.
