SALARY:
- Social Worker III: $70,008 ($5,834.00/month), BU:13, SR-20, Step J (includes $122/month shortage differential)
- Social Worker IV: $80,004 ($6,667.00/month), BU:13, SR-22, Step K (includes $242/month shortage differential)

JOB SUMMARY:
The Social Worker position is primarily responsible in identifying and providing for each patient’s social, emotional and psychosocial needs and the continuing development of the patient’s full potential before or during their stay in long-term care. This position is assigned to the Social Work Services Department at Hilo Medical Center, which is part of the East Hawaii Region facilities under Hawaii Health Systems Corporation.
MINIMUM QUALIFICATIONS:
Education and Experience:
- Social Worker III: Bachelor’s Degree in Social Work and at least one year of social worker experience, or Master’s Degree in Social Work with no social work experience.
- Social Worker IV: Master’s Degree in Social Work (License preferred) and at least one year of social work experience.
Additional Experience:
- Health care experience preferred.
- Must meet any state and/or Federal requirements for the position.
- Basic understanding of computer technology, including email use.
- Must have ability to read, write and follow oral and written directions at a level necessary to accomplish the job.
- Must be able to relate positively and favorably to patients and families and to work cooperatively with other associates and personnel of other community agencies.
- Must have knowledge of State and Federal guidelines.
- Must have demonstrated skills in communication, documentation, assessment and social work methods techniques.
- Must meet all local health regulations, and pass post-employment physical exam if required. This requirement also includes drug screening, criminal background investigation, and reference inquiry.
ESSENTIAL JOB FUNCTIONS
Departmental
- Develop a comprehensive social history assessment, a psycho-social assessment and discharge planning through review of patient information, personal contact with patient/family and interdisciplinary team members.
- Actively participate in interdisciplinary meetings and patient/family/caregiver meetings to promote communication, education and support between family members, facility staff and community agencies.
- Provide education and assistance to patients and families on topics of Advance Directives, Providers Orders for Life Sustaining Treatment, Durable Power of Attorney for Healthcare and Living Wills.
- May initiate legal proceedings such as guardianships and the completion of Social Summaries as indicated.
- Ensures all options for the patients’ plan of care have been communicated to the patient/family and members of the healthcare team and are documented as necessary to ensure continuity of care.
Safety
- Comply with all safety policies, regulations and requirements regarding fire and life safety.
- Comply with all safety policies regulations and requirements involving employee safety and patient safety. This includes but is not limited to: a zero tolerance for harassment and violence; following infection control procedures; following safe patient handling as required with your specific job position.
- May be required to assist in the evacuation of patients during emergency situations.
- Meet the general health requirements set forth by the policies of this facility.
SUPERVISORY APTITUDE: Demonstration of an aptitude for the performance of supervisory duties. Supervisory Aptitude may be demonstrated through completion of regular or special assignments involving responsibility for some supervisory functions; by serving as a group or team leader; by completion of training courses in supervision accompanied by application of supervisory skills in work assignments; or by favorable appraisals by a supervisor stating the possession of interpersonal skill indicative of supervisory potential.
MEDICAL/PHYSICAL EXAMINATION REQUIREMENT: Offers of employment will be contingent on successfully passing a pre-employment examination, which includes drug screen and other regulatory medical requirements such as, but not limited to, tuberculosis (TB) screen.
COVID-19 VACCINATION REQUIREMENT: COVID-19 vaccination is a condition of employment for Hawaii Health Systems Corporation. After receiving a conditional offer of employment, all prospective new HHSC employees are required to be fully vaccinated against COVID-19. New employees are required to provide confirmation of their full vaccination status by presenting their CDC vaccination card or VAMS Certificate prior to their start date.
