Job Description


Under general supervision of the Director of Social Work, a Social Worker Level III or higher, and the unit coordinator, the Social Worker Level II provides:

SUMMARY OF DUTIES AND RESPONSIBILITIES
  • Focuses on providing social work service for all Medical Social Work clients, with specialization in support for HIV patients and patients with significant disabilities.
  • Assesses for concrete service and mental health needs for all clients
  • Provides case management support
  • Conducts mental health support, including crisis intervention, short-term psychotherapy, and psychosocial assessments
  • Refers clients to appropriate in-house and/or community resources, as necessary
  • Maintains record-keeping, documentation, and scheduling in a timely and professional manner - scheduling program, clinical notes, case management notes, Medical Social Work productivity logs, and any database collection information
  • Attends weekly clinical supervision with LCSW
  • Collaborates with a multidisciplinary team of medical doctors, nurse practitioners, physician assistants, nurses, PCAs, PHAs, and clerical staff to provide effective and efficient support for patients
  • Supervises social work interns, and obtains SIFI certification
  • Updates staff of community services and best practices as they arise
  • Bolsters relationships with outside agencies specialized in care for the Gouverneur patient population
  • Attends departmental meetings, conferences, social work meetings, Grand Rounds, and ongoing education requirements
  • Completes H+H administrative requirements, i.e. trainings, recertification, etc .
  • Participate in QA and UR activities
  • Provides clinical supervision hours for LMSWs seeking a Social Work License once LCSW licensure is obtained

Minimum Qualifications


For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.

CERTIFICATION(S)/LICENSE(S): LMSW/LCSW

EDUCATIONAL LEVEL: MSW

KNOWLEDGEABLE IN:
Preferred experience or education in health care, social services in an unserved community and working with individuals who have mental illness.

EQUIPMENT/MACHINES OPERATED:
Ability to operate typically office equipment; copier, fax, scanner, etc.

COMPUTER PROGRAMS/SOFTWARE OPERATED
Microsoft programs, Outlook. Experience with scheduling programs and electronic medical records helpful.
