The Social Service Director plans, organizes, develops, and directs the overall provisions of Social Services in accordance with current Federal, State and local standards, guidelines and regulations, and facility established policies and procedures. The Social Service Director is responsible for ensuring that social services are provided to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident on an individual basis.
Essential Job Functions include but are not limited to:
- Complete Admission Packets timely, accurately and completely
- Participate in resident assessments, provide background information and social history, as identified in the MDS process
- Interview resident/responsible party to obtain necessary information, maintaining confidentiality at all times
- Provides information to resident/responsible party as to Medicare, Medicaid and other financial assistance programs available to the resident
- Involves the resident/responsible party in planning objectives and goals for the resident
- Participates in regularly scheduled reviews of resident discharge planning
- Assists in providing in-service education and orientation for employees regarding Resident Rights and social service needs of the residents
- Encourages socialization within the facility to provide a healthy life enrichment for each resident
- Provides onsite tours of Facility to Prospective Residents and Families

Job Type: Full-time
Pay: $33,000.00 - $40,000.00 per year
