PURPOSEProvides in-home nursing services to pediatric patients under the supervision of a registered nurse. Works closely with interdisciplinary team to ensure high quality patient care and outcomes.

KEY RESPONSIBILITIES
-
Performs prescribed individualized patient care to patients as delegated.

Participates in patient case conferences as appropriate.

Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.

Observes, documents and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

Provides nursing care in a way that considers patient's privacy, comfort and safety.
-
Demonstrates the ability to instruct patients in the home according to the plan of care.

Performs procedures based on the Agency's policies and procedures, including but not limited to all educational requirements.

Accepts telephone orders from physicians in accordance with scope of practice.

Documents patient information in a clear, legible, timely and accurate manner according to policy.Maintains all licenses/certifications as required.

Participates in the Agency's continuous quality improvement activities.

May participate in selected research projects as appropriate.

Utilizes infection control practices with the delivery of patient care services.

Works collaboratively with peers and other healthcare workers.

MINIMUM QUALIFICATIONS
-
A graduate from an approved school of practical nursing.

Must be licensed in VT & NH

REQUIRED LICENSE/CERTIFICATIONS/REGISTRATIONS SKILLS (Tracked at the department level)

LPN License in VT and NH

Schedule: Per Diem, Rotating, Days, Nights, Weekends
