Primary Responsibilities:

To provide expertise and coordination of all aspects of health care in the home for patients under the direct orders of licensed/credentialed physician or nursing plan of care when no skilled need is present. The Home Care Nurse is responsible for assessing and documenting patient's physical, emotional and socio-economic needs at each visit.


Specific Job Duties - includes, but not limited to:

  • Operate efficiently in Devero and Microsoft Office Suite.
  • Educate patients and caregivers regarding process, nutrition, medication, and self-care.
  • Provide accurate and timely documentation.
  • Perform direct client care including but not limited to tube management, venipuncture, chest auscultation, wound care, assistance with pain management, ostomy, and catheter care.
  • Communicate information regarding patient's condition or needs appropriately to implement and facilitate patient's plan of care.
  • Evaluate outcomes of the implemented plan of care.
  • Develop individualized patient care plans to address specific care or services to be provided to the patient per physician's orders.
  • Participate in on-call coverage and scheduled visits to meet patient needs during non-office hours.
  • Maintain home care services' supplies and equipment according to the State Board of Health requirements and agency policy.
  • Maintain established agency productivity standards.
  • Attend and participate in available seminars, meetings, projects and workshops to maintain and increase knowledge base.
  • Attend on-the-job and in-service training.
  • Attend clinical and all other meetings accordingly.
  • All other duties and tasks as assigned

Experience:

  • 1-3 years of Home Care experience
  • Demonstrated ability to multi-task.
  • Ability to work and communicate professionally with patients, medical partners, employees, and supervisors.
  • Ability to work independently and as part of a team.


Required:

  • Graduate professional school of nursing; Associates Degree in Nursing.
  • Current State of Minnesota LPN or RN License in good standing.
  • Current CPR Certification for Health Care Providers
  • Good judgment and decision-making skills.
  • Strong attention to detail.
  • Technologically proficient and driven by accuracy.
  • Provide dependable transportation for patient visits.
  • Act as a role model of Advanced Medical Home Cares values and mission.
  • Regular attendance and flexibility with scheduling necessary.


Working Conditions:

  • Ability to sit, stand, type, see, hear, communicate verbally and in writing.
  • Ability to drive a vehicle.
  • Ability to complete job tasks within 32-40 hours per week as stated in offer.
  • Contact with patients under wide variety of circumstances.
  • May be exposed to patient elements, unpleasant elements (accidents, injuries/ illness, pets, insects, communicable diseases) and may be subject to varying and unpredictable situations.
  • Handles emergency or crisis situations.

**We are hiring for coverage primarily in the south metro including the cities of: Bloomington, Savage, Shakopee, Burnsville, Apple Valley and Lakeville.**
