Registered Dietitian Job at Northwest Health Services Inc
Described Duties:
1. Work cooperatively with Northwest Health Services (NHS) providers and personnel to carry out the goals and objectives of NHS according, to governing body requirements and established internal policies and procedures.
2. Treat others respectfully whether it is a patient, fellow employee, member of management, customer, vendor or visitor to our premises.
3. Contribute to department goals and standards as well as serve as a diabetes and nutrition resource to patients, community and other health care professionals.
4. Advise, assess and support patients in making specific health behavior changes. This includes goal setting, problem solving and implementation of diabetes self -management care/support.
5. Conduct approved curriculum in a group setting, managing the group process to facilitate positive personal adjustment to diabetes and its treatment.
6. Manages and coordinates all nutrition education material to be used within DSMES and MNT programs.
7. Provides one-on-one or group nutrition education/counseling.
- For a variety of health concerns/preventative wellness including but not limited to heart health, diabetes, kidney disease, weight loss, malnutrition, failure to thrive, formula and breastfeeding education and pediatric overweight/obesity.
- Diabetes education to follow and support AADE 7 Behaviors.
8. Evaluate and provide individual assessment of diabetes patients educational needs, (medication, meal plans, exercise, insulin injection, glucose monitoring. etc.) and facilitates behavior change congruent with treatment goals.
9. Assess, educate, advise, counsel and problem solve in individual or small group appointments or over the phone. Follow up as appropriate.
10. Works with staff to develop educational goals, learning objectives and a plan for content and teaching methods for program participants.
11. Develops individualized follow-up plans with each DSMES and MNT participant.
12. Continually evaluates effectiveness of program by measuring attainment of learning objectives.
13. Conducts follow-up assessments upon completion of DSMES program curriculum.
14. Documents intake data, educational plan, educational services received by participant and results/goals achieved.
15. Engages in a team approach to provide services and collaborations with other professionals or team members when needed.
16. Identifies when a program participants needs are outside the scope of the instructors practice and expertise, works to arrange for additional services to meet needs.
17. Maintain effective record keeping systems for follow-up members, along with documentation of class attendance and learning goals.
18. Responsible for keeping the patients primary provider informed about the patients status and providing ongoing instruction to the patient.
19. Directs continuous quality improvement processes to measure DSMES program and MNT standards and identifies opportunities for improvement.
20. Maintains 15 hours of continuing education annually specific to nutrition, MNT, diabetes, diabetes related topics and behavior change and self-management education strategies.
21. Consistent and timely review of education materials and proposition for improvement based on current standards/evidenced based research.
22. Ensures program is following the Scope of Practice, Standards of Practice and Standards of Professional performance for Diabetes Educators and Register Dietitians.

24. Must comply with the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
25. Provide nutritional counseling not limited to diabetic patients.
26. Perform related work as required.
Essential Job Functions:
1. Must be able to talk, see, hear, react, stand, kneel, and walk.
2. Possess cognitive skills necessary to read and interpret a variety of instructions furnished in oral or written form.
3. Must demonstrate tact, patience, and good communication skills.
4. Focus on specific behaviors which are essential for improving the diabetes patient's health and quality of life.
5. Effective in collaboration and therapeutic alliance with individuals to perform MNT.
6. Experience/Expertise in nutrition as it relates to self-management of diabetes and other chronic diseases.
7. Must respect and maintain confidentiality at all times.
8. Maintain professional appearance and demeanor at all times.
Qualifications:
1. Graduate of an accredited school.
2. Healthcare professional who is a Registered Dietitian or eligible to become a Registered Dietitian within 3 months of hire, diabetes experience preferred.
3. Licensed to practice in the state of Missouri
4. Certification by the national certification Board for Diabetic Educators preferred.
5. Familiarity with American Diabetes Association recognition guidelines.
6. Current CPR certification.
7. Possess skills in counseling and motivational interviewing.
8. Capable to lead and manage groups.
9. Must demonstrate basic computer skills.
10. Possess light typing skills, computer knowledge preferred. Must demonstrate accuracy, proficiency, and efficiency.
11. Must possess ability to effectively present information and respond to questions from patients, and other individuals encountered in the course of employment.
12. Must possess and maintain a valid Drivers License, and proof of insurance for personal vehicle used for business.
