The QIDP is a psychologist, social worker, or human services professional with specific higher education accomplishments and experiences that enable him/her to ensure that the services received by individuals we serve in our Community Day Services and residential programs are consistent with the individual's wishes, needs, and desires (as documented in that individual's implementation strategy), and monitoring agency requirements (if any).
The QIDP provides habilitation management services necessary to ensure a individual's progress towards the outcomes set for them through the personal planning process, and that the outcomes specifically identified in the client’s personal plan are measured and considered in the development of the client’s implementation strategy. The QIDP collects the raw data necessary to evaluate program effectiveness, efficiency, and stakeholder satisfaction.
The QIDP ensures Rimland’s compliance with requirements legally-mandated by the State of Illinois Department of Human Services (DHS) related to program participants served, and the delivery of programs and services to them. Consequently, the QIDP must be knowledgeable about and proficient in the application of all DHS requirements with which Rimland must comply, including DHS Rules 50, 115, 116, 119 and HCBS guidelines.
Qualifications
The QIDP must possess experience working directly with individuals with intellectual disabilities or other developmental disabilities (see specific experience requirements listed below), must satisfy all requirements to be recognized by DHS as a QIDP, must possess excellent verbal and written communications skills, must have demonstrated an ability to work well with others as part of a team, must have demonstrated an ability to relate well to program participants and others within program participants’ circles of support.
Experience
Must have at least one (1) year of experience working directly with individuals with intellectual disabilities or other developmental disabilities; be either a psychologist, a social worker, or a human services professional; and satisfy all applicable educational requirements listed below.
Position outline: The Community Day Services QIDP will assist in facilitation and coordination of community outings for the individuals we support based on their individual calendars. Participate and document individual responses to the activities. Assist in the development of the individual calendars based on their identified outcomes. Complete monthly summaries of the individual's progress towards meeting identified outcomes. Participate in annual personal plan meetings and provide summary updates specific to individual progress towards outcomes. Assist in training staff to ensure CDS outcomes are carried out as identified in the individual's plan. Ensure all supplies needed to carry out identified outcomes are available to the individuals. Lead monitoring visits with ISC agencies at CDS & CILA program. Complete individual assessments as assigned. Other duties as assigned.
Education
The QIDP’s educational background will vary depending upon their career choice. If a social worker, he/she must possess a bachelor’s degree from a college or university or graduate degree from a school of social work accredited or approved by the Council on Social Work Education or another comparable body. If a human services professional, he/she must possess a bachelor’s degree in a human services field, including, but not limited to sociology, special education, rehabilitation counseling, or psychology from an accredited program (must provide original copy of diploma and sealed transcript).*Please note: Degrees or transcripts issued by a foreign educational institution must be evaluated for U.S. grade or degree equivalence by a vendor qualified to make such evaluations. Transcripts can be verified by a vendor that is a member of the National Association of Credential Evaluation Services (NACES) or other association qualified to make these determinations.
Work Shift
Monday-Friday 9 a.m.- 6 p.m.
Pay Rate
$24/hour

- Valid Driver’s License
- Three (3) business references
- Ability to pass Test of Adult Basic Education
- Eligible to work in the United States
- Ability to pass criminal background checks
- Submit proof of COVID-19 vaccination (individuals who have received the first dose will be permitted to begin the interview process),
