GENERAL DESCRIPTION OF DUTIES
The primary purpose of this position is to serve as a regional public health consultant for the regional based public health special projects in Region 8. The service area includes Bladen, Brunswick, Columbus, Duplin, New Hanover, Onslow, Pender, Robeson, and Sampson Counties. Special projects include the ARPA COVID-19 Public Home Regional Workforce (including Care Management for High-Risk Pregnancies [CMHRP] and Care Management for At-Risk Children [CMARC] programs) and ED Regional Prevention Support Teams. This position should have a considerable knowledge of advanced communication skills as well as the ability to assess, design, and plan specific service models to encompass a variety of health issues and cultural differences for high risk and costly patient groups. Each of the projects have a specific Agreement Addendum (AA) to include scope of work and deliverables as well as specific performance measures, reporting requirements, performance monitoring, and quality assurance. Program planning, policy-making, and development of regional-wide public health services will be under the direct supervision of the Duplin County Public Health Director in collaboration with Region 8’s Public Health Directors. Additionally, this position will provide supervision and oversight of the Accounting Specialist II position, Regional Prevention Support Team Infection Prevention Coordinator, and Regional Workforce Development Director.
SPECIFIC DUTIES AND RESPONSIBILITIES

75% Care Management for High-Risk Pregnancies (CMHRP) and Care Management for At-Risk Children [CMARC] programs· Conduct a gap analysis to identify current challenges and opportunities for a shared services model for the CMHRP and CMARC programs within its region.· Conduct an environmental scan of counties within its region to determine readiness and acceptability of shared services model for the CMHRP and CMARC programs.· Provide a minimum of two training sessions for new and existing staff within its region on care management best practices, team building, and potential integration of community health workers.· Develop pre- and post-training surveys and utilize them to measure participant comprehension of training topic and inform future training sessions.· Create a shared service model proposal for CMARC/CMHRP programs within its region including a monitoring and evaluation plan.
10% ARPA COVID-19 Public Home Regional Workforce· Maintain a Regional Workforce Development Director to provide expertise, leadership, and collaboration across Region 8 to fulfill the AA specific activities in this AA.· Prepare and complete a COVID-19 PH Workforce Grant Work Plan (“Work Plan”).· Produce a gap analysis to identify regional staffing and training shortfalls.· Manage a training program to enhance the skills and expertise of Region 8’s LHD staff.· Supports regional efforts to recruit, hire, and train personnel to address projected COVID-19 response needs and regional activities.· Maintain a Regional Public Health Workforce Leadership Team comprised of Region 8’s Local Health Directors and Human Resources staff or designees to advise the Regional Director on the content and execution of the Public Health Workforce Work Plan.· Strengthen the 7-Public Health Infrastructure Foundational Capabilities to address local public health priorities deriving from COVID-19.

· Provide monthly progress updates on the staff recruitment and hiring plan to include metrics on applications received and successful candidates based on race, ethnicity, and gender as well as on number of successful hires in relation to hiring goals. · Provide monthly progress updates on the regional training plan, including the number of training sessions to include demographics of race, ethnicity, and gender of participants in each training sessions.
10% ED Regional Prevention Support Teams
· Engage and maintain proactive working relationships with administration of long-term care facilities (LTCFs) on the prevention, early detection, and infection preventions strategies for COVID-19 and other communicable conditions of concern.
o LTCFs include nursing homes/skilled nursing facilities, adult care homes/assisted living facilities, independent living facilities, family care homes, and behavioral health facilities.
· Maintain and provide programmatic oversight of a Regional Prevention Support Team (RPST) to include a Regional Prevention Coordinator and additional prevention trainers based upon the number of long-term care facility beds in Region 8.
o Training for the RPSTs will be provided by the Communicable Disease Branch (CDB) in collaboration with UNC SPICE.
o Participate in CDB-sponsored meetings coordinated by the CDB Program Manager.
o RPSTs will collaborate with UNC SPICE to develop additional resources based on needs assessments and identified gaps.
· Maintain a Regional Prevention Support Leadership Team consisting of representatives from a majority Region 8’s local health departments to routinely meet in-person or conference call to discuss the needs of the region.
o Region 8 RPST’s Regional Prevention Coordinator will
§ provide monthly reports to this leadership team and to each local health director in the region.
§ work with the Regional Leadership Team to carry out the developed action plan and maintain networked partnerships with each LTCF.
§ evaluates the progress of the RPST via monthly evaluations of the RSPT dashboard metrics.
o Ensure the RPST uses evidence-based infection control procedures consistent with applicable CDC, CMS and HHS guidance to bolster infection prevention knowledge and practices, mitigating and preventing health threats like COVID-19 and all other infectious diseases, thereby reducing morbidity and mortality
MARGINAL FUNCTIONS
While the following tasks are necessary for the work of the unit, they are not an essential part of the purpose of this position and may also be performed by other unit members.
5% Other Duties
· Serves as a team member during outreach projects such as flu vaccine season and preparedness exercises.
· Serves on the management team during disasters for staffing of emergency shelters.
· Attends meetings and other duties as assigned by health director.
MINIMUM TRAINING AND EXPERIENCE
Education/Experience
· Master’s Degree in Nursing, Public Health, or Public Health Administration or related area and two years of experience in public health or epidemiology; or
· Graduation from an accredited School of Nursing and four years experience in or related to public health nursing or epidemiology.
Additional Training/Experience Desired
· Formal training in biological, chemical, and radiological terrorism agents.
· Experience in public health, pediatrics, maternity, immunization, family planning, primary care, child health, communicable disease law, and grant writing.
· Completion of Principles and Practices of Public Health if does not have BS in Nursing
· Public Health Nursing experience in team leading, program development, and data analysis.
SPECIAL REQUIREMENTS
· Licensed to practice as a Registered Nurse in the State of North Carolina by the North Carolina Board of Nursing.
· Valid NC Driver’s License and reliable means of transportation for work assignments
· Certified in basic CPR life support
