PURPOSE
The purpose of the position is to provide caregiving support and respite services including education, counseling and group support, outreach, and resourcing to caregivers. The Family Caregiver Specialist responds to referrals to determine the need for Family Caregiver Support or Respite Service, provides information, training and counseling to consumers and caregivers, acts as a link to caregiver services and provides referrals as appropriate to a variety of assistance programs. The Family Caregiver Support Program (FCSP) is funded by the Administration on Community Living and administered by the State Office of Aging and Disability Services as well as through other grants, donations, and State funds. The Family Caregiver Specialist is supervised by and reports directly to the Aging & Disability Resource Manager.
Duties include:
· Responds in a timely way to all referrals. Provides information and assistance services and discusses individual needs and preferences by phone or in-person
· With the consumer and caregiver, identifies options and assistance needed, provides information, including authorizations of respite services, training needs, and appropriate referrals to services and programs available within the caregiver’s area.
· Provides counseling and ongoing caregiver support in person and via phone. Supports caregivers through acknowledgment, encouragement, empowerment and decisional support.
· Links families to needed formal and informal resources for both the caregiver and the care recipient.
· Provides timely, appropriate, and relevant information specific to the caregiving situation. Initiates educational opportunities for caregivers tailored to meet their individual needs.
· Develops, coordinates and provides caregiver training on relevant topics including provision of Savvy and other structured, evidence-based workshops.
· Facilitates support groups and other mechanisms for caregivers to feel supported
· Uses creative strategies and materials to reach caregivers not currently receiving services and to develop opportunities to present and distribute educational materials to individuals, groups, businesses and other members of the community.
· Attends meetings of community groups and makes presentations to large and small groups, agencies, health fairs, etc. Provides training and technical assistance to individuals, groups and businesses to identify, engage and connect family caregivers to supportive services and resources.
· Contributes to the development of relevant content for Spectrum Generations web site and other public education materials.
· Collaborates with ADRC, Community Center Staff and other Spectrum Generations staff to assure that these employees are knowledgeable and can make appropriate referrals to FCSP and Caregiver Respite.
· Collaborates with community groups (i.e. TRIAD) to supplement their efforts to identify and serve family caregivers; advocate with individuals, agencies, community service providers and family members on behalf of the caregiver.
· Reports cases of suspected abuse, exploitation, or neglect, to Adult Protective Services.
· Markets, coordinates, refers, and administers various respite programs including the Alzheimer’s Respite and Kinship programs.
· Consistently enters consumer data into the Agency electronic data base and maintains appropriate documentation in a timely manner.
· Processes, reviews for accuracy, and authorizes respite care reimbursement requests, obtains concurrence from Program Administrator and forwards to Finance Service for payment.
· Adheres to Spectrum Generations policies and procedures
· Performs other duties as assigned to meet organizational goals.

Essential Functions of the Job:
· Working knowledge of biological, psychological, and social aspects of aging; the impact of disabilities and illnesses on aging, human behavior and effects of psychological stresses.
· Working knowledge of caregiving techniques, Alzheimer’s, dementia and related diseases, public and private community resources and referral programs available to caregivers; fee for service options; the Aging network; and long term care issues.
· Principles and techniques of interpersonal communication, support group facilitation, interviewing principles, and public speaking.
Skill and ability to:
· Effectively communicate with consumers and families including those in crisis or with special needs, professional staff and the general public with tact and courtesy.
· Knowledgeable of the proper use of modern office equipment, including telephone system, facsimile, photocopier, audio/visual equipment, computers and other office/technical equipment used in the execution of job duties; and must be able to utilize this equipment with or without reasonable accommodation.
· Ability to enter data accurately and expeditiously into electronic data base.
· Excellent organizational and communication skills (verbal & written).
· Ability to maintain confidentiality of client information.
· Interview individuals in a variety of settings to identify and assess problems.
· Prioritize multiple needs and determine appropriate information, referrals and resources.
· Establish and maintain cooperative working relationships with public and private agencies and other health care professionals to promote cooperation and information sharing.
· Understand, interpret and explain Federal and State rules and regulations.
· Think creatively and apply complex information to a variety of situations

· Make effective presentations to large and small groups.

Education/Training/Experience: Knowledge Required by the Position
1. BS Social Services or related area.
2. LSW or ability to obtain LSW-conditional preferred
3. 3-4 years progressively more responsible work experience in community relations.
4. Must possess a State of Maine driver's license in good standing
Working Conditions and Physical Demands:
Occasional travel required. Travel, often by personal vehicle expected. May be subject to emotionally upset consumers or family members. Light physical demands including standing, walking, prolonged sitting, writing and time at a computer. Works in an OSHA Exposure Category 1.
