HVCS a Division of Cornerstone Family Healthcare is actively recruiting for a Prevention Specialist to join our growing team in Hawthorne, NY.

RATE OF PAY/SALARY: $40,000. annually

WORK LOCATION(S): Hawthorne, NY

STATUS: Full Time

JOB SUMMARY:

  • Under the supervision of the Program Supervisor, the Prevention Specialist is a member of the Lower Hudson Syringe Exchange Program (SEP) team, a mobile van-based outreach operation, which provides harm reduction supplies and referrals to people who inject/use drugs (PWID/PWUD) in Westchester, Putnam, and Rockland Counties.
  • Conduct outreach to individuals at high risk at the community level as well as within drug rehabilitation centers, shelters, soup kitchens, and other community-based organizations to publicize program services and recruit potential clients.
  • Provide SEP services to PWID within Westchester County.
  • Provide referrals and referral follow-up to drug treatment, entitlements, medical treatment, and HIV/HCV/STI services where applicable.
  • Complete data entry into HVCS’ database system (AIRS), data should be entered correctly for each intervention and service. Data must be entered in a timely manner
  • Observe and abide by HIPAA and the HIV Confidentiality Law.
  • Maintain ongoing relationships with administrators and staff at community-based resources and local police where the program will provide services.
  • Responsible for the oversight and proper maintenance of the agency vehicle.
  • Responsible for providing services and maintaining program inventory of STSEP services, providing syringes and harm reduction services to priority population.
  • Conduct screening, intake, enrollment, and follow-up of clients in the program and enter client data into the agency’s database in a timely manner consistent with agency policies.
  • Review, develop and continually assess the demands for services in the communities where SEP provides services and develop programming as needed.
  • Provide referrals to drug treatment, entitlements, medical treatment, and HIV/HCV/STI services where applicable.
  • Responsible for quality assurance activities, including maintaining client charts within program standards and ensuring compliance with goals and objectives in the annual workplan.
  • Complete the mandated continuing education trainings as selected and scheduled by the Program Supervisor.
  • Participate in individual and team supervisions and attend department meetings.
  • Assist in program coverage as needed.
  • Attend and participate in monthly department, All Staff, and other required meetings.
  • Be familiar with Cornerstone policies and procedures and the Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other related duties as assigned.
Requirements:
  • Bachelor’s degree in education, health, human services, or related field with experience in outreach, HIV/HCV or STI counseling and prevention.
  • Or an Associate's Degree with two years of experience in peer education, HIV/AIDS counseling and/or disease prevention will be considered.
  • Or High School Diploma with four years of experience in peer education, HIV/AIDS counseling and/or disease prevention will be considered.
  • Minimum of one year experience working with PWUD, knowledge of HIV/AIDS, harm reduction, mental health and/or community resources.

Travel/Time Requirement: Must have a valid, unrestricted driver's license and independent means of transportation for frequent travel to locations throughout the assigned county as well as to the offices of Cornerstone
