Population Health Specialist Job at Red Rock Behavioral Health Services
Summary
Provides support and coaching to promote good physical health and wellness to adults, youth and their families through individual and group activities. Supports outpatient through adult, youth and family support groups
Essential Functions
- Assists with individual physical wellness plan development, implementation, assistance and support
- Conducts support, exercise, and other groups for adults, children and for adult caregivers and/or identified natural supports
- Provides prevention, support, and group activities to Population health and Community individuals
- Assists adults, youth and their family regarding behavioral, interpersonal, communication, self-help, safety, substance use decisions, and specific goal-setting and problem-solving activities
- Helps clients and families understand Crisis Plans and Comprehensive Care Plan process
- Helps clients and families understand medications side effects and possible effects on overall health and wellness
- Provides direct clinical services to adults/youth on an individual, group, and family bases in the office and community settings to teach behavioral symptom-management techniques in order to alleviate and manage symptoms not reduced by medication and to promote person growth and development by assisting clients to adapt to and cope with internal and external stresses
- Helps support and foster local youth and family support groups
- Networks with community leaders, agencies and organizations to support the work of the local youth group
- Ensures the child/youth voice in family, treatment, community and agency activities/meetings
- Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment
- Works as Population Health Team member for clients within Red Rock’s treatment team
- Attends staff meetings, workshops, and seminars to learn agency policy, rules, regulations, and procedures; participates in on-going in-service training as well as required external training
- Supports and assists with the facilitation of Care Plans for consumers within the Red Rock Treatment Teams
- Performs other duties as assigned
Qualifications
- High School Diploma or General Equivalency Diploma required, Bachelor’s Degree in mental health or health-related field preferred
- At least two years relevant experience preferred
- Strong one-on-on and group communication skills
- Oklahoma Department of Mental Health and Substance Abuse, Certified Case Manager I or Wellness Coach, preferred
- At least 21 years of age
- Valid OK driver’s license, clean driving record, transportation
- Excellent verbal and written communication skills
- General skill in exercising independent judgment, tact, patience, and understanding; maintaining effective working relationships with clients, other staff, and the community
- Must complete all in-service and external training requirements
