Principal Responsibilities:

The Sam Field Older Adult Center, serving adults aged 60+, is a program of Commonpoint Queens, a multipurpose social service agency. Our mission is to sustain and enhance the quality of family and individual life for all members of our community, regardless of religion, ethnic or cultural background, national origin, gender identity or expression, sexual preference, age, or disability, within a pervasive and inclusive environment that facilitates individual growth, meets individual needs and builds community. We accomplish this mission through a broad range of social, educational, cultural and recreational services and programs that reach community members of all ages and backgrounds.

Minimum Qualifications:

DUTIES AND RESPONSIBILITIES

• 
  Functions as part of a team providing support group services and memory enhancing activities for older adults who are aware of their dementia diagnosis
• Meets with potential new participants, and their caregivers, to assess their appropriateness and readiness to participate in a memory loss program and/or caregiver support group
• Registers new clients, completes initial and follow-up assessments and documents accordingly in the agency's client management system and/or the Department for the Aging's (DFTA) database (STARS Peer Place), as appropriate
• Co-facilitates Early Stage group sessions and documents attendance in client management system
• Facilitates Caregiver support groups and/or provides individual counseling to Caregivers and documents services provided
• Maintains updated electronic participant records
• Assists withlunch service
• Participates in program development
• Other duties as assigned

EDUCATION, EXPERIENCE, AND REQUIRED COMPETENCIES

• Master's Degree in Social Work
• Experience working with a geriatric population, preferably those affected by memory loss
• Computer savvy; experience utilizing Google Docs/Sheets, Microsoft Office applications and client management systems for documentation helpful

SPECIFICATIONS FOR PHYSICAL REQUIREMENTS

• Proof of vaccination against the COVID-19 virus is required
• Ability to stand for one hour

  Schedule: part-time, 18 hours per week, Tuesday through Thursday, 9:00 AM to 3:00 PM
• Salary:$25.24 per hour
