JOB SUMMARY:
The OT2, PRN goes to the patients home and provides occupational therapy services to patients who have impairments, developmental disabilities, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes under occasional guidance. Actively collaborates with the health care/educational team to meet patient care, outcomes and system improvement objectives.
This position is for VUMC adult therapy program serving patients with a wide range of diagnoses and ages. The therapist will provide evaluations, treatments and education to patients.

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.

KEY RESPONSIBILITIES:
- Analyzes comprehensive information from formal and informal assessment that is pertinent to the patient's care and management to determine a rehabilitation treatment diagnosis.
- Collaborates with stakeholders in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the rehabilitation continuum.
- Applies appropriate, evidence-based interventions to ensure effective outcomes and re-evaluates performance to assess patient's progress towards goals.
- Employs strategies to promote successful return to home, community, and school/vocational environments.

Position Requirements:
- Bachelor Degree
- 1 year relevant experience
Licensure, Certification, and/or Registration (LCR):
- Licensed Occupational Therapist Required
Strongly Preferred by Department:
- CPR certification
- Home Health experience
- Excellent communication skills
- Highly organized
- Flexible

- Medium Work category requiring exertion up to 50lbs of force occasionally and/or up to 20 of force frequently and/or up to 10 of force continually to move objects.
