Welcome to the WELLBOUND family!
WELLBOUND Certified Home Health Agency is currently seeking interdisciplinary therapists to serve our pediatric and adults with special needs in their home environment. There is paid training, active leadership and an abundance of resources! We are looking for therapists able to commute to Far Rockaway (11691) for our pediatrics and adult special need population.
- Physical Therapists
- Occupational Therapists
- Speech Language Pathologists
Responsibilities:
- Design and implement individualized treatment to address deficits and optimize function and client centered goals.
- Provide quality, personalized, evidence-based care, and proven interventions.
- Document all treatments provided via EMR.
- Educate and train HHAs (Home Health Aides) to carryover skills addressed in therapy with the support of our unique Thrive program and home exercise programs.
- Provide effective and timely communication with interdisciplinary staff, families, and caregivers.

Qualifications:
- Valid NYS Professional License

Pay Rates:
PT/OT/SLP: $85.00 Per Visit
