We are looking for an experienced Occupational Therapist to join our home health team. The ideal candidate will have a strong background in physical and rehabilitation medicine, as well as experience working with geriatric patients. The successful candidate will be responsible for providing therapeutic services to patients in their homes, assessing patient needs and developing treatment plans, and educating patients and families on the use of assistive devices.
Responsibilities:
- Provide home health occupational therapy services to clients.
- Assess patient needs and develop individualized treatment plans.
- Evaluate patient progress and modify treatment plans as needed.
- Instruct patients in the use of adaptive equipment and assistive devices.
- Educate patients and families on safety, injury prevention, and home exercise programs.
- Provide physical and rehabilitation medicine services to geriatric patients.
