Ravenwood Specialty Care is now seeking a Memory Care Program Coordinator:
The primary purpose of this position is to plan, organize, develop, and direct the overall operation of the memory care program in accordance with current federal, state and local standards, guidelines, regulations, established facility policies and procedures and as directed by the Administrator.
Responsibilities:
Ensure that activities are individualized and customized based on the resident’s prior lifestyle (occupation, family, hobbies), preferences and comforts.
Establish a systematic approach to memory care providing a space and environmental cues that encourage physical exercise, decrease exit-seeking behavior and reduce extraneous stimulation (i.e., seating areas along a walking path, room with a calming atmosphere, using music, light, etc.).
Plan and coordinate an interdisciplinary therapeutic program schedule.
Keep abreast of current research, new programs and community resources which may benefit residents and families.
Ensure the memory care team has the resources to provide individualized care as identified by the facility and resident assessments.
Develop and implement an orientation and training program for dementia and memory care units including pre/post-training evaluations and competency assessments.:
Education:
Must possess, as a minimum, a bachelor’s degree in Recreational Therapy (CTRS), Nursing (BSN), or Social Work (BSW) from an accredited school.
Experience:
Must have, as a minimum, two (2) year(s) of experience in a supervisory capacity in a hospital, nursing care facility or other related medical facility.
Experience working with residents or patients with memory or cognitive impairment recommended.
Specific Requirements:
Must be able to read, write, speak, and understand the English language.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. Knowledge of the GEMS model.
Must be a supportive team member, contribute to and be an example of team work and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
Must have patience, tact, cheerful disposition and enthusiasm as well as the willingness to handle residents based on whatever maturity level in which they are currently functioning.
Must possess the ability to seek out new methods and principles and be willing to incorporate them.
Must not pose a direct threat to the health or safety of other individuals in the workplace.:
