BASIC PURPOSE:
To perform clerical and electronic duties for the nursing department and any other department necessary for the collection and completion of accurate medical records for each resident. To collect, complete, store, retrieve, maintain security of these records in accordance to the organizations policies and procedures, and in compliance with regulatory standards.
MAJOR WORK ACTIVITIES:
1. Assures effective communication with other staff members/disciplines with regards to medical record maintenance.
2. Answers and assists with telephone communication, assists with the front desk, assists with transportation, and helping to orient new employees based on the needs of the facility.
3. Making sure that all coding is current, resolved and newly documented diagnosis are documented.
4. Send out MARS for physicians signatures. These records should be mailed out every six months, and to track the MARS so that they are being returned to the facility.
5. Maintenance of all medical records, closed and opened.
6. Check mail at all times to make sure that all information is being addressed in a timely manner.
7. Assist with providing resident information /documentation for long term insurance policies.
8. The scanning of pertinent medical records for maintenance and upkeep of the residents charts. Making sure that all records are scanned, and placed in the Matrix in a timely manner.
9. To make sure that the records are maintained and protected from loss or destruction and unauthorized use of manual records.
10. Weights and vitals signs are to be documented by the middle of each month for each resident.
11. Maintain system for filing, retention and destruction of overflow/thinned records
12. Maintains facility policies and standards of practice, maintains routine audits and focus audits, QA concerns included but not limited to quarterly and closed record chart system.
13. Making necessary copies and assist with mailing services
14. The making of necessary spreadsheets and to assist with payroll.
15. The appropriate tracking and update of all FL2’s, LOC and care plans.
16. Assist with nursing duties whenever necessary.
17. Maintains all master forms and management forms for development review and reproduction of these forms.
18. Communicates issues and concerns with the Resident Care Director or Administrator, and reports to Quality Assurance Coordinator whenever necessary.
19. Performs specific duties and responsibilities as per Resident Care Director and Administrator.

Requirements:KNOWLEDGE, SKILLS AND WORKING CONDITIONS:
- Work is performed indoors with controlled temperatures.
- Minimal exposure to blood, body fluids and infectious diseases
- Physical exertion required no more than 60% of the time. Physical exertion also is described as standing, stooping, kneeling and bending.
- Intermittent lifting and carrying of objects weighing between 30-40 pounds and pushing of objects weighing at least 100 pounds.
- Exposure to some noise and stress
- Ability to communicate and problem solve effectively
- Ability to perform simple mathematic problem solving for inventory/ordering purposes
- Medical terminology and computer skills required
- Sufficiently mobile to push carts and equipment, with frequent standing and stooping
- Ability to evaluate and appropriately respond to verbal and nonverbal communications from residents
- Physical job demands may be subject to possible modifications to reasonably accommodate individuals with disabilities.
Education: High School Diploma or GED
Licensure/ Certification: Certification not required
Experience: At least 1 year of medical and clerical experience.
