LPN- Licensed Practical Nurse Job at Yours Truly Inc
Skilled Nursing LPN is responsible for fulfilling the needs of patient health/medical care in the client's home consistent with Yours Truly, Inc. culture, mission values, philosophy, policies and procedures. The nurse is responsible for following Physician ordered Plan of Care for medication administration and treatment, obtaining supplemental doctors' orders as needed and supporting client to attend medical appointments. Nurse is expected to monitor, document, and report changes in general client health and condition to assigned RN manager or Director of nursing. Accurate and thorough daily documentation of services is required along with submission of weekly reports within required time frames. It is expected the Nurse acts with professionalism, maintains credentialing report and participate in on-going training to maintain and enhance skills.
Requirements:- Must be 18 years of age or older, a legal citizen of The United States and maintain a valid Virginia Driver's License.
- Basic fluency in oral and written English.
- Have satisfactorily completion of nursing education program with current, valid LPN or RN license issued by The Virginia Board of Nursing.
- Provide written disclosure statement identifying pending criminal charges and convictions. Submit to Criminal Background Check and be clear of barrier crimes of abuse/neglect convictions.
- Have reliable vehicle transportation to work and have a reliable backup transportation plan.
- Possess the physical ability without restrictions to perform mechanical or physical lifts and/or transfers according to individual client needs and protocols.
