Education/Experience: Must be a graduate of an accredited school of Nursing; Must have valid registration as an LPN in the State in which applying for employment with PNES or OOPNES Certification in Pharmacology; must have completed Board of Nursing approved I.V.; Therapy course preferred; must have current AHA Healthcare Provider training (BCLS); One year experience in medical/surgical nursing and/or area of specialty preferred. Job Summary: The LPN is responsible to the Department Director, and its clients for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse. Essential Functions: · Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes. · Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards as observed by the registered nurse. · Assist in data collection on the Admission Data Record. 1. Contribute to the development of the patient/significant other plan of care based on the identified patient problems/needs. 2. Carry out physician orders accurately, directly and indirectly. 3. Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient/ significant other identified problem/needs. 4. Document pertinent observation and intervention on appropriate record. 5. Perform selected tasks related to patient care and the patient's environment in a safe manner. · Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes. · Perform limited I.V. Therapy procedures on those patients receiving peripherally inserted Intravenous fluids, as delegated by and observed by the RN. · Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback. · Give punctual change of shift report consisting of pertinent and relevant information for assigned patients. · Provide patient/significant other health teaching as delegated by and observed by the RN. · Use available written and AV materials specific to patient condition or disease. · Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussion. · Participate in staff development programs and meetings as requested. · Maintain resident confidentiality; treat residents with kindness, dignity and respect; know and comply with Resident's Rights rules; and, promptly report all resident complaints; accidents and incidents to supervisor. · Assist in the orientation/training of new employees. · Consistently work in a positive and cooperative manner with fellow employees. · Assist other employees in the performance of their assignments as requested seeking out opportunities to help rather than waiting to be asked. · Recognize the need for variations in staffing and volunteer to fill open shifts. · Take direction and initiate actions (cross/additional training) that will allow the assumption of cross[1]functional duties to ensure seamless patient care. · Demonstrate flexibility to perform duties wherever volume deems it necessary within the system. · Demonstrate sound judgement by taking appropriate actions regarding questionable findings or concerns; Investigate and follow through on unusual orders or requests for service or information; Follow proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur; Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service. · Contribute to the achievement of Performance Improvement and participate in PI activities utilizing the problem-solving methodology of F.A.D.E. for performance improvement. · Consistently evaluate work and determine if further steps are needed to meet client expectations. · Ensure compliance with regulatory standards. Consistently demonstrate ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary. · Minimize non-productive time and fill slow periods with activities that will enable you to prepare to meet the future needs of the system (education, organizing, housekeeping, assisting others). · Organize job functions and work area to be able to effectively complete varied assignments within established time frames. · Consistently demonstrate ability to take the initiative to make decisions/choices without direct supervision · Attend required annual in-service programs. · Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals. · Comply with the system's infection control policies and procedures including Body Substance Isolation (BSI) and the Exposure Control Plan which reflects OSHA Bloodborne Pathogens Standard to ensure a safe working environment for self and others. · Ensure that the system is operated in an efficient and cost effective manner by adhering to system and departmental attendance policies as documented on the employee attendance record. · Demonstrate adherence and support of system-wide service standards as evidence by observation and client feedback including complaint management standards, dress code standards, and safe practices standards. Critical Physical, Perceptual or Mental Abilities Required of Essential Functions: · Intermittent physical activity including walking, standing, sitting, lifting and supporting patients. · Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment. · Incumbent will be required to work at any facility and be responsible for own transportation. · Incumbent will be scheduled based on operational need. (rotate shifts, standby, AA, on-call) · Incumbent may be required to work extended periods of time at a video display terminal (VDT). · Incumbent may experience traumatic situations including psychiatric, dismembered and deceased patients. · Demonstrates effective verbal and written communication skills. · Utilizes effective time management skills · Ability to demonstrate competency skills as required. · Ability to read and write in a legible and understandable manner. · Must speak and understand English. · Positive attitude toward the elderly. Marginal Functions of Position: · Participates in the interviewing process for new staff as requested. · Participate in training and education of staff as requested. · May perform additional duties as requested by supervisor. Reporting Relationships: · Reports to the Charge Nurse or designated person. COVID-19 considerations: Must Be COVID-19 Vaccinated.
