Responsibilities of the Life Skills Counselor: The Community Oriented Recovery and Empowerment Services (CORE) Life Skills Counselor is responsible for providing support, skill building, and education to individuals who have a diagnosis of mental illness and their family members. Services are person-centered community based, and occur in settings where individuals live, work, learn and socialize. Services may be offered individually or in groups. Duties include:

  • Provide home-based or community-based therapeutic interventions to assist individuals in reaching recovery goals. Use a trauma-informed care and harm-reduction approach to support individuals as they achieve stability and functional improvements. Develop and maintain trusting and supportive relationships with individuals, families, and community providers and support
  • Engage individuals in a person-centered Intake & Evaluation process to identify recovery goals, strengths and resources, barriers and needs, preferences for service delivery, and how the CORE service will be used to support attainment of their goals.
  • Assist individuals in identifying specific behavioral health barriers that impact daily living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community participation.
  • Partner in the development and implementation of strengths-based Individual Service Plans consistent the with individual’s values, culture, beliefs, and goals. Update Individual Service Plans on an on-going basis.
  • Provide support and counseling focusing on psychoeducation, skill building, rehabilitative counseling, money management, medication management, and work or school readiness to individuals.
  • With permission, engage and educate family members with the intention of supporting individuals’ recovery goals. Education focuses on recovery concepts, trauma, self-care, crisis intervention and prevention, advocacy, and systems navigation. Engage Families in skill development and support with respect to creating safe environments, developing coping and problem-solving skills, increasing healthy family communication skills and facilitating family and friends support groups.
  • Act as a Representative Payee to help clients and former clients of Mohawk Opportunities’ programs manage their finances, pay their bills, and develop the skills needed to gradually assume responsibility for these tasks on their own.
  • Collaborate with all community providers including Managed Care Organizations, Health Home Care Management Agencies, Behavioral Health treatment agencies, and other CORE and HCBS service providers.
  • Responsible for all documentation related to assessing, enrolling, and providing services to eligible individuals using the collaborative documentation model. This model engages the individual in the completion of all documentation and occurs at the end of each visit. All documentation must be completed in a professional and timely manner.

Salary and Schedule of the Life Skills Counselor:

  • $20.26/hour. $42,140.80/annual equivalent
  • 40 hours per week. Monday-Friday. DAY TIME HOURS AND NO ON-CALL. Some flexibility with early morning hours as clients might have early morning medical appointments.

Skills/Qualifications for the Life Skills Counselor:

  • Associate’s degree in human service required; Bachelor’s degree in human services preferred.
  • One year of experience working in a behavioral health setting.
  • Ability to be flexible, have excellent organizational skills, and ability to follow through with multiple tasks.
  • Must have a strong commitment to the rights and ability of each personal with a mental illness or substance use disorder to make their own decisions and choices, live in the community, work, and have access to continuous support and services to enhance wellness and recovery.
  • A valid and insurable NYS driver's license. If using your personal vehicle for business purposes, you must maintain the NYS minimum insurance coverage.
