Job Description:

The Community Health Center, Inc. Lead Dietitian is responsible for the management of all dietitians in the coordination and implementation of all phases in the Nutrition Care Process and in providing Medical Nutrition Therapy, as well as all nutrition programming at CHC, Inc. These areas include, but are not limited to, nutrition assessment, care planning, monitoring, follow up, and evaluation, education, and counseling for a variety of nutrition-related medical conditions. The lead dietitian is responsible for assuring compliance with the Joint Commission, DPH, NCQA, ADA and other standards.

Duties and Responsibilities

  • Along with all dietitian duties, the lead dietitian will:
    • Approve time cards, PTO in Novatime, and coverage schedules (as necessary) for all dietitians
    • Review and assess case presentations made by RD/RDNs at monthly nutrition meetings.
    • Sets annual goals and objectives for dietitians
    • Implements and communicates all policy and regulatory changes from the DPH, AND, and CAND
    • Explores ways to expand CHC’s nutrition and health education services
    • Works collaboratively with CMO and CFO on productivity expectations and the annual budget
    • Complete 6 month and annual performance appraisals and improvement reports in a timely manner
    • Assist the Chief Medical Officer and Medical Quality Improvement Committee in reviewing and updating nutrition-related policies and procedures
    • Works collaboratively with Human Resources and the CMO on recruitment, interviewing, hiring, training and orientation of new Dietitians, as well as staff retention
    • Provide ongoing review and assessment of the Nutrition Care Process and MNT provided by the dietitians and dietitians/diabetes educators (dual licensed) based on their patient’s medical needs
    • Develop and oversee nutrition-related peer reviews
    • Provide clinical nutrition services as required of Level 1 dietitians
    • Creates and updates the clinical expectations for RDs, on an annual basis, at minimum
    • Complete weekly chart reviews for new RD for a minimum of 4-6 weeks and review this with employee
    • Review all nutrition-related education documents created by the dietitians for accuracy and patient-appropriateness prior to distribution
    • Train RD and RD/CDE staff in appropriate nutrition related documentation utilizing ECW
    • Convene meetings of the nutrition staff monthly and create agenda for such meetings
    • Work collaboratively with the Site Directors, state-wide, and on site leadership to coordinate nutrition templates, visits and referral processes
    • Assist in grant writing, program development, and participation in CHC health-related activities
    • Promote the tenets of Patient-Centered Medical Home and Primary Care Medical Home care delivery, specifically focused on the role of RD and RD/CDE staff members in the care team
    • Encourage the adoption, uptake and spread of CHC clinical efforts
    • Encourage community involvement of RDs and RD/CDEs such as health fairs, food demonstrations, and participation of CHC-sponsored events (Heart Health Day, Diabetes Day, etc.)
      • Assist other RDs and RD/CDEs in the development of these programs as needed
    • Support RDs and RD/CDEs in new ideas that create community nutrition and health awareness
    • Facilitate Food Smart and Fit groups at their respective sites
    • Assist RDs in recruitment, preparing, monitoring and evaluating, and running these groups as needed Monitors RDs and RD/CDEs productivity as needed, while maintaining their own, based on CHC’s standard expectation for RD employees

Required Qualifications

Education and Experience:
  • Baccalaureate degree in Food and Nutrition, Dietetics, Nutrition Sciences, Human Nutrition or comparable majors required.
  • Successful completion of a CADE accredited dietetic internship of no less than 900 hours under the supervision of a Registered Dietitian.
  • Master’s degree in Clinical Nutrition, Public Health, Community Nutrition, or comparable major preferred
  • Two years in an acute care or outpatient clinic where Medical Nutrition Therapies are provided with one year supervisory experience preferred; experience may be waived for specific positions and experiences.
  • Degreed, Licensure, and/or Certification:
  • Registered Dietitian (RD) by the Commission on Dietetic Registration
  • Certified Dietitian/Nutritionist (CDN) by the Connecticut Department of Health
  • MS, MPS, or MA preferred
