What will you do as a Integrated Health Care Specialist at Easterseals?

You'll have the opportunity to work alongside a team of healthcare providers as you perform customer service and conduct initial screenings of individuals served. You'll file, audit, and maintain charts and co-facilitate team meeting minutes regarding team-based care and other population health.

In this position, you'll have the opportunity to flex your analytical muscles as you ensure data integrity, review and share data reports and outcomes of team services. What Does Easterseals Need From You?

• Be a high school graduate or have a certificate of successful completion of the General Educational Development
• Have three (2) years of responsible full time clerical
• Excellent customer service
• Ability to make general decisions in accordance with departmental policies and
• Knowledge of modern office equipment, practices, and
• Knowledge of modern record keeping management and
• Knowledge of grammar, spelling and
• Ability to follow complex oral and written
• Computer literacy and competency with Microsoft Office Professional package including demonstrated proficiency in Access, Excel, PowerPoint, and Word

Education

• High School or better in Primary Source
