POSITION DESCRIPTION | START OF CARE NURSE

The RN is the “gatekeeper of care” and develops a patient care plan that is customized to each patient; working with the patient, the family and the physician to best meet the care and wellness needs for each individual patient. Come find how rewarding this type of nursing can be!

This position is accountable for the start of care to admit new clients to home health. This includes coordination of in-home nursing care, including direct client care, patient/family education and transitions of care. Must evaluate client nursing needs and develop plans of care and goals appropriate to client assessment per physician orders. The RN will be accountable for planning, implementing, evaluating and communicating all phases of nursing care for assigned clients. Skill set must include delegation of additional disciplines of patient care to team members according to skill level, patient acuity, and plan of care. The RN must possess excellent communication skills; be skillful in mentoring and teaching; and approach tasks with a positive and flexible attitude. Participation in team meetings, case conferences and in-service education is required.

QUALIFICATIONS:

Minimum of one year RN experience (home health care experience a plus)

Background check required

LICENSE OR CERTIFICATION: RN, Drivers License, CPR Certified
