Position Title: MDS Coordinator

Overview: The MDS Coordinator ensures that the Minimum Data Set (MDS) and care planning processes are completed according to established standards and policies. The MDS Coordinator is responsible for ensuring the uniform application of the MDS and care planning programs, policies and procedures. The MDS Coordinator will abide by and follow established policies and procedures as outlined by the State and Federal Regulations.

Essential Job Functions:

• Ensures that the MDS/Care Planning process are completed in a timely manner, and that appropriate staff, including but not limited to C.N.A. nurse, PT/OT/ST, Activities, Dietary, Social Services, family and resident attend the care plan meeting.
• Review charts to ensure that all disciplines have provided appropriate and required documentation.
• Performs all new admissions assessments and/or reviews assessments when completed by other RN/LPN staff.
• Interacts with all disciplines and the residents and family members to collect information regarding residents.
• Ensures that the MDS is completed in a timely manner.
• Writes nursing care plans for all residents/updates as needed.
• Communicates with the residents, family, friends, HC Staff, pharmacy, and other as necessary.
• Complies with date sensitive time frames for computer data entry.
• Transmits MDS data to the State in a timely manner (no less than weekly).
• Ensures all residents are scheduled for care plan meetings including new admissions ongoing OBRA requirements and emergent family conferences are coordinated with SW.
• Prioritizes MDS assessment completion to maximize facility revenue / reimbursement.
• Coordinates with billing to ensure accuracy in billing log.

You Have:

• Graduation from an accredited School of Nursing.
• LPN or Registered Nursing license in good standing.
• Experience with the CMS Resident Assessment Instrument process.
• Must have geriatric nursing experience.
