Responsibilities:
The Central Community Health Partnership is looking for a Director of Behavioral Health Services to oversee integrated care coordination and care management for individuals enrolled in the Central Community Health Partnership’s Behavioral Health Community Partner (CCHP/BHCP) Program. The Director of Behavioral Health Services will formulate clinical decisions to manage serious and persistent mental illness and substance abuse issues that promote health and wellness, as well as will have in depth knowledge of the community and medical insurances in the area. They will directly supervise a team of Clinical Care Managers, Registered Nurses and Care Coordinators who provide integrated care management in collaboration with individuals served, community partners, medical and behavioral health providers.

The ideal candidate will have experience in complex case management and population health management as well as the ability to work with a high degree of independence in a dynamic environment. The Director of Behavioral Health will have the ability to manage through influence and foster a collaborative work environment.

Other Key Responsibilities:
  • Strong relationship management with internal and external stakeholders while representing CCHP and Open Sky Community Services with professionalism.
  • Lead the care coordination/care management staff providing regular supervision, performance appraisal and professional development.
  • Lead in ACCS/DMH relationship with internal and external stakeholders. Lead BHCP Consumer Advisory Board and provide leadership and management for the Behavioral Health Community Partner Program.
  • Assist in the collection of service outcomes, annual analysis and other reporting and initiatives as required
  • Participate in the development and implementation of the program’s strategic priorities and goals.

Qualifications:
  • Licensed as a registered nurse (RN), Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC), required.
  • Minimum of five years of experience working with adults with serious and persistent mental illness and substance use disorders, required.
  • Minimum of two years of supervisory experience, required.
  • Extensive experience with local resources and navigating health, behavioral health and/or substance abuse treatment systems.
